Introdução: A incidência de transtornos depressivos no puerpério é alta. De 10 a 16% das puérperas preenchem critérios para o diagnóstico de depressão-pós parto (DPP). O pré natal psicológico (PNP) é uma prática complementar ao pré-natal tradicional, voltado para maior humanização do processo gestacional, que se propõe profilática. Objetivos: relacionar os níveis de ansiedade e depressão gestacional ao risco de desenvolver DPP e avaliar a eficácia do PNP na prevenção à DPP. Metodologia: realizou-se uma pesquisa-ação em um recorte longitudinal. Participaram da pesquisa 76 mulheres de uma maternidade pública, em Brasília, que se dividiram em dois grupos para fins comparativos: o grupo intervenção, composto por 47 gestantes que participaram do PNP e o grupo controle composto por 29 gestantes que não participaram do PNP. Os instrumentos utilizados foram: Inventário Beck de Depressão (BDI) e ansiedade (BAI) e Escala de depressão pós –parto (COX). Procedeu-se a uma análise quantitativa dos dados, para avaliar a associação entre a chance de DPP e os níveis de ansiedade e de depressão gestacional, nos dois. Resultados: o teste utilizado foi o Quiquadrado, que mostrou existir evidências de associação entre essas variáveis. No grupo intervenção, a depressão gestacional moderada ou grave foi diagnosticada em 10 gestantes (21,27%), contra 6 gestantes (20,68%) no grupo controle. Já ansiedade gestacional foi diagnosticada em 21 gestantes (44,67%) do grupo intervenção contra 9 gestantes (31,02%) do grupo controle. Somando-se os valores de depressão e ansiedade gestacional de cada grupo, obtém-se 31 gestantes ou 65,94% de incidências no grupo intervenção e 15 gestantes ou 51,7% no grupo controle. Ou seja, o grupo intervenção apresentou um percentual maior de gestantes em risco de desenvolver a DPP. Entretanto, ao contrário do esperado, apenas 5 puérperas ou 10,64% apresentaram a DPP contra 13 puérperas ou 44,82% no grupo controle. O Qui-quadrado mostrou que para o grupo intervenção o nível de ansiedade e depressão gestacional não está associado com o fato da gestante ter depressão ou não. Assim, mesmo que a gestante seja classificada com nível de depressão gestacional e ansiedade grave ou moderado, o risco de DPP é muito menor para quem participou do PNP. Conclusão: pode-se inferir que o PNP atuou como um fator de proteção minimizando os riscos que a ansiedade e a depressão gestacional podem trazer para aumentar a chance da DPP. Defendemos que o PNP é importante instrumento psicoprofilático que deve ser implementado como uma política pública em contextos da saúde no Brasil.
