Introdução: A epilepsia, síndrome clínica neurológica grave e comum, é caracterizada pela recorrência de episódios, convulsivos ou não. É uma doença cerebral crônica causada por diversas etiologias e caracterizada pela recorrência de crises epilépticas não provocadas. Estima-se que a prevalência mundial de epilepsia ativa esteja em torno de 0,5%-1,0% da população e que cerca de 30% dos pacientes sejam refratários, ou seja, continuam a ter crises, sem remissão, apesar de tratamento adequado com medicamentos anticonvulsivantes. Normalmente as convulsões são tratadas com antiepilépticos tradicionais, tais como ácido valpróico, lamotrigina, carbamazepina e clobazam. Apesar dessas vantagens, a eficácia e os efeitos colaterais destas drogas novas são ainda pouco conhecidos, particularmente em crianças. Com o progressivo aumento da esperança média de vida e a incidência de doenças prolongadas e incuráveis, houve um aumento significativo do número de doentes reavivando o interesse terapêutico pela cannabis. Objetivo: Avaliar evidências científicas existentes para uso dos produtos derivados de cannabis no tratamento da epilepsia refratária em crianças e adolescentes. Metodologia: A busca por evidências foi realizada nas bases de dados eletrônicas Cochrane Library/Willey, MEDLINE via PubMed, EMBASE, BVS e LILACS. A estratégia de busca foi construída utilizando vocabulário controlado representativo dos componentes “população” e “intervenção” da pergunta estruturada, além de palavras de texto a fim de sensibilizar a estratégia. Para a escolha da melhor evidência disponível priorizou-se a seleção de estudos do tipo ensaios clínicos. Resultados: Um dos estudos envolvendo 162 pacientes analisou redução no número de crises convulsivas e segurança terapêutica, durante o uso de Cannabidiol (CBD) em associação a outros anticonvulsivantes de uso comum. Outro ensaio com 120 pacientes, randomizado, demonstrou redução no número de eventos epilépticos no grupo que fez uso de CBD, porém um maior número de eventos adversos, em detrimento ao grupo placebo. O terceiro estudo, com 13 pacientes, demonstrou redução no número de crises durante o uso de CBD, analisando a interação farmacológica entre o CBD e o fármaco Clobazam. Conclusão: A recomendação deste parecer a favor da tecnologia é fraca, pois são necessários outros ensaios clínicos randomizados, com maior número de participantes e maior duração do estudo
