A importância de analisar a evolução das Doenças Crônicas Não Transmissíveis (DCNT) no último século decorre da insuficiência de estudos que resgatem o caráter histórico das mudanças ocorridas na sociedade brasileira, as quais determinam os processos de transição demográfica e epidemiológica, num contexto de relevantes desigualdades sociais. No campo do conhecimento, apesar do avanço dos métodos de análise baseados nos modelos epidemiológicos de associação entre riscos e problemas de saúde, surge a necessidade de um redirecionamento na busca de identificação e de comprovações mais consistentes dos determinantes sociais dessas enfermidades, a partir da construção de modelos conceituais integrativos, que dêem conta da complexidade dos seus níveis de determinação. Os modelos lineares de explicação, como algumas teorias acerca da transição epidemiológica, bem como os que se propõem a explicar componentes específicos do processo de determinação (biologia humana e estilo de vida) mostram-se insuficientes para explicar a determinação das DCNT. Considerando o período correspondente ao século XX, adotamos uma matriz de determinação social como forma de apreender os contextos político, econômico e de ocupação do espaço urbano em diferentes níveis de determinação (macrodeterminações, microdeterminações e determinação individual/de grupos). Na análise temporal de mortalidade, estimamos modelos de regressão linear simples e consideramos os grupos das Doenças do Aparelho Circulatório (DAC), das Neoplasias Malignas e o Diabetes mellitus. Observamos, como conseqüência das transformações ocorridas no país no decorrer do século XX, que ocorre a passagem de um padrão arcaico de transição epidemiológica, em que enfermidades crônicas não transmissíveis ainda não são reconhecidas como questões importantes do ponto de vista da saúde pública, para um padrão de desigualdades, no qual essas enfermidades assumem a supremacia no perfil de morbimortalidade da população. As conclusões indicam a necessidade de ponderação acerca das possibilidades e limites das intervenções pontuais, de caráter conjuntural e compensatório, bem como a relevância de intervenções intersetoriais, integrais que visem a modificar as condições de vida do conjunto da população
