Informações sobre despesas segundo doença são crescentemente valorizadas na elaboração de políticas de saúde. Monitorar gastos com HIV/AIDS, malária, tuberculose e dengue segundo um marco contábil único auxilia na compreensão do vulto e da origem do financiamento para combate a essas doenças e pode subsidiar o debate sobre seu financiamento adequado no âmbito do SUS. Consolidar estimativas de gastos públicos com tuberculose, dengue, malária, HIV/AIDS segundo as esferas de governo para os anos de 2010 a 2013, destacando a participação (%) nas despesas do SUS e a contribuição do financiamento federal. Estudo de custo de doença, baseado na perspectiva de incidência, com recortes seccionais anuais de despesas do SUS com dengue, malária, tuberculose e HIV para o Brasil nos anos de 2010 a 2013 utilizando o arcabouço metodológico do System of Health Accounts. Foram usados dados orçamentários (SIGA Brasil para governo federal e SIOPS para governos estaduais e municipais), de utilização de serviços ambulatoriais e hospitalares (SIA e SIH), sobre medicamentos e insumos (SIASG e SICLOM), além de dados sobre incidência da doença (SINAN e SIVEP), seguida da definição de produtos característicos e conexos para cada doença contidos nessas bases e consolidação das despesas a eles relacionadas. Despesas do SUS com as doenças somaram, a valores correntes, entre R$ 3,3 bilhões (2010) e R $ 4,67 bilhões (2013). HIV correspondeu a 1,0-1,1% das despesas do SUS, dengue a 0,7%-1,2%, tuberculose a 0,3% e malária a 0,1-0,3%. Entre 2010-1, apenas HIV teve taxas de crescimento nominais das despesas comparáveis às do SUS; para 2011-2, gastos para todas as doenças cresceram menos que o SUS; em 2012-3 apenas gastos para tuberculose e dengue cresceram mais que o SUS. A participação federal variou de 59-73% nas despesas com malária, 60-74% no HIV, 33-39% na tuberculose e 45-69% no dengue. Cresceu na malária, caiu na tuberculose e expressivamente no HIV e oscilou na dengue segundo anos epidêmicos. A tendência à redução dos gastos nessas doenças relativamente às do SUS ocorreu em um período onde não havia ainda clara retração do financiamento do SUS e indica baixa visibilidade desse gasto em situações não epidêmicas. Malária foi a que mais perdeu e tuberculose a que mais manteve financiamento. Houve queda expressiva nos gastos federais para HIV. Epidemias de dengue geraram aumento expressivo da participação federal nos gastos.
