PROVISÃO FEDERAL EM VOLUME E GASTOS DE INSULINAS REGULAR E NPH NO BRASIL, ENTRE 2009-2015

A insulina é central para o controle do diabetes mellitus. No Brasil, a provisão insulina pelo Governo Federal se dá através de compras licitatórias pelo Departamento de Logística em Saúde do Ministério da Saúde (DLOG-MS) e pelo reembolso a estabelecimentos do varejo farmacêutico contratados diretamente, de acordo com o fornecimento de insulina (Programa Farmácia Popular do Brasil-PFPB). O objetivo deste estudo é analisar a provisão de insulina, em volume e gasto, pelo Governo Federal brasileiro, entre 2009-2015. Os dados de aquisições via DLOG-MS e o reembolso via PFPB foram utilizados como proxi de consumo. O volume e gasto de insulinas NPH e Regular (R) foram obtidos através de bases administrativas oficiais. Os dados foram analisados de acordo com a provisão total do Governo Federal e de acordo com as provisões do DLOG-MS e do PFPB. Os dados também foram apresentados de acordo com o tipo de insulina. O volume da provisão foi calculado e apresentado em número de doses diárias definidas (DDD)/1000 habitantes/dia. Os gastos foram corrigidos a valores de 2015 e convertidos a dólares americanos (USD). A razão gasto:volume (DDD/1000 hab/dia) foi calculada. O total de 133,5mil unidades de insulinas foram fornecidas pelo Governo Federal entre 2009-2015, com gasto de 748.467.888,99 USD. Houve crescimento do volume no período. A NPH representou maior proporção de volume e gasto no total de aquisições. PFPB apresentou aumento de total volume e gastos, com os gastos apresentando aumento maior que aquele para volumes. Volumes e gastos do PFPB foram maiores em 2015 e 2014, respectivamente(3,29DDD/1000hab/dia; 74,86 milhões USD). DLOG-MS foi o maior provedor de insulina, com provisão de maior volume total em 2014(13,04DDD/1000hab/dia) e maior gasto total em 2013(155,51 milhões USD). A razão gasto:volume foi 3 vezes maior para o PFPB que para o DLOG-MS. Os volumes e gastos do Governo Federal brasileiro com insulinas mostraram crescimento durante o período. A insulina NPH aparentemente é o principal driver de volume das aquisições federais. A provisão via PFPB está crescendo, mas ainda não é capaz de cobrir a do DLOG-MS. A maior razão gasto:volume no PFPB pode indicar preços mais altos nesta forma de provisão do que os praticados pelo DLOG-MS.
