Comparados àqueles em aleitamento materno exclusivo (AME), recém-nascidos que recebem outros alimentos além do leite materno têm maiores chances de óbito por infecções. Os hospitais credenciados à Iniciativa Hospital Amigo da Criança (IHAC) se comprometem a estimular o aleitamento materno exclusivo durante a internação hospitalar e não oferecer fórmulas infantis sem justificativa médica. Estimar a prevalência de aleitamento materno exclusivo na internação hospitalar após o parto e analisar a associação entre parto em Hospital Amigo da Criança e esta prática. Inquérito nacional de base hospitalar com amostragem complexa representativa de todos os nascimentos ocorridos em hospitais no Brasil entre 2011 e 2012. Foram excluídos bebês ou mães com impedimentos para iniciar o aleitamento materno, resultando em amostra de 21.086 sujeitos. Além da exposição principal, foram investigadas características maternas, da atenção pré-natal, da atenção ao parto e do bebê. Razões de chance ajustadas foram obtidas por modelo de regressão logística que considerou o desenho complexo da amostra e alfa de 1%. A prevalência de aleitamento materno exclusivo durante a internação hospitalar foi de 76%. Mães que tiveram seu filho em hospital credenciado à IHAC, público (OR=2,11;1,20-3,73) ou privado (OR=3,66;1,72-7,8), tiveram maiores chances de amamentar seus filhos exclusivamente na maternidade, bem como mães que tiveram parto normal (OR=2,24;1,76-2,84). Bebês que nasceram prematuros (OR=0,49;0,39-0,62) e aqueles que foram internados em UTI neonatal (OR=0,44;0,22-0,88) tiveram menores chances do desfecho. Para cada hora a mais entre o parto e a entrevista, a chance da mãe ter seu filho amamentado exclusivamente no hospital diminuiu em 3% (OR=0,97;0,96-0,98). Observou-se que o credenciamento à IHAC teve associação positiva ao AME na internação hospitalar após o parto, tanto no setor público quanto no privado, evidenciando o papel fundamental desta iniciativa para a promoção e proteção do aleitamento materno. Além disso, a associação negativa entre parto cesáreo agendado e o aleitamento materno exclusivo traz mais evidência aos atuais esforços para diminuir esta prática nociva no Brasil.
