O cão doméstico é considerado o principal reservatório da Leishmaniose Visceral (LV) em áreas urbanas, mas a identificação de gatos infectados por Leishmania sugerem a possibilidade desses animais também atuarem como reservatórios. Para a caracterização de uma espécie como reservatório há a necessidade de evidências epidemiológicas na coocorrência entre as espécies e a infecção em questão. Este estudo tem o objetivo de acessar evidências científicas existentes sobre a associação entre exposição a gatos e a ocorrência de Leishmaniose Visceral em humanos e cães. Uma revisão sistemática foi realizada segundo recomendações do PRISMA (The Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Foram avaliados estudos publicados até 25 de outubro de 2017 nas bases PubMed/Medline, Scopus and LILACS (Latin American and Caribbean Health Sciences Literature), sem restrição de linguagem. Os critérios de elegibilidade para a leitura do texto na íntegra foram: i) Estudos epidemiológicos de desenhos: Transversal, coorte, caso-controle e ecológico; ii) Desfecho: Leishmaniose visceral em humanos ou cães; iii) Exposição: Presença ou interação com gatos domésticos. Os aspectos metodológicos dos estudos foram analisados segundo a iniciativa STROBE. Dentre os seis estudos abarcando LVC, um apresentou uma alta chance de ocorrência LVC na presença de gatos, um mostrou relação inversa entre a presença de gatos e LVC, e quatro foram inconclusivos. Entre os quatro estudos abordando LVH, três foram inconclusivos, e um apresentou associação positiva entre gatos e LVH em pacientes que realizaram transplante renal. A inconsistência dos resultados, associado à fragilidade metodológica dos estudos analisados não permite uma conclusão consistente de que existe coocorrência entre exposição a gatos e LV. Estudos com metodologia mais robusta devem ser realizados para elucidar sobre o papel dos gatos na transmissão de LV em humanos e cães.
