No atual contexto sanitário das comunidades ribeirinhas do Médio Solimões no Amazonas, frente à organização político-territorial do SUS, devem ser considerados os processos de transformações socioambientais dos territórios com a criação de Unidades de Conservação Ambiental, cujo o atendimento às demandas sociais é complexo e conflituoso porque envolvem várias instituições no apoio a gestão local. Analisar o acesso à saúde das populações ribeirinhas que vivem na Reserva de Desenvolvimento Sustentável Mamirauá, Estado do Amazonas. A pesquisa é quantitativa e os dados foram obtidos por questionário da “Escuta Itinerante: acesso das populações do campo e da floresta ao SUS”, em agosto de 2016. Para amostra (N=231) utilizamos a prevalência de 18,5% de indivíduos que não foram atendidos por médico na Escuta ao SUS do país. Também foram consideradas na seleção as comunidades que receberam e as que não receberam apoio comunitário de uma organização social, e que fossem localizadas nos rios Solimões e Japurá, sendo sorteadas oito comunidades. Os questionários foram digitados no SPSS e analisados a partir das quatro dimensões do acesso proposta por Fekete (1996) que são: geográficas, econômicas, organizacionais e culturais. As rotas e ciclos fluviais são elementos geográficos impeditivos. O tempo de deslocamento a UBS varia entre 20 minutos (ribeirinhos de São José de Maiana) e 4 horas (Porto Braga) que somam-se aos custos de R$ 100,00 com a rabeta, sendo aspectos econômicos limitativos. No tocante a dimensão organizacional, a porta de entrada apresenta problemas. Para 76,6% dos ribeirinhos a assistência comunitária se limita ao ACS. Verificou-se uma iniciativa de Ambulanchas promovida por uma outra organização social. A dificuldade de compreensão da língua e do jargão médico de profissionais estrangeiros e brasileiros do Programa Mais Médicos foi referida como aspecto limitativo da dimensão cultural. O esforço para acessar a saúde nas sedes de Fonte Boa e Uarini nem sempre tem desfecho satisfatório, pois 25,5% dos ribeirinhos não conseguiram atendimento médico, e no geral, em 67,5% das situações o paciente não teve acesso à medicação prescrita. Em virtude dessas questões, a avaliação do serviço de saúde mostra-se negativa, visto que, na nota de zero a cinco atribuída pelos entrevistados, 54,1% avaliaram em menos de 2 pontos os serviços.
