Esta pesquisa de tese de doutorado se constitui de uma análise de experiências de inclusão social no processo de reforma psiquiátrica no Brasil, especialmente através dos projetos de arte-cultura e trabalho que vêm sendo criados e desenvolvidos nas últimas décadas, a partir das lutas no campo da saúde mental, que têm permitido o surgimento de formas inovadoras de relação com a loucura e a diversidade. A partir da Análise Genealógica de Michel Foucault, busca-se refletir sobre as rupturas com o paradigma psiquiátrico que podem ser operadas através das inovações nas experiências de diversidade cultural da reforma psiquiátrica. A pesquisa de campo foi realizada no Brasil e na Itália, e incluiu 14 entrevistas com fundadores e participantes de 8 projetos artístico-culturais no Brasil, no Rio de Janeiro e São Paulo, nas áreas de teatro, música, artes plásticas, bloco carnavalesco e uso de linguagens multiartísticas. E também 9 entrevistas com fundadores e participantes de importantes projetos de inclusão social e cooperativas de trabalho na Itália, em Trieste, Milão e Roma. Por fim, foram incluídas na análise 3 exposições de arte em conexão com a questão da loucura: as exposições “Mais que Humanos” (SP) e “Lugares do Delírio” (RJ), no Brasil; e a exposição “Progetto LABRYS”, no Parco Basaglia em Gorizia, na Itália. Uma série de questões fundamentais são colocadas para o campo da saúde e da saúde mental: o problema da colonização biomedicalizante do discurso e da prática e a continuidade da tendência à burocratização e à padronização dos modos de intervenção; mas também o uso de múltiplas linguagens artísticas ou hibridismos de forma inovadora ou não-convencional, com a dissolução de fronteiras rígidas entre as artes; o princípio do co-envolvimento e da horizontalidade, fundado na quebra de hierarquias e na ruptura com o discurso especialístico; e o uso da arte rompendo a sua captura como “mercadoria” – o que leva a uma desinstitucionalização da arte e a um repensar os conceitos de cuidado e de saúde. Em síntese, os projetos de arte-cultura e os projetos de trabalho, economia solidária e cooperativas sociais se constituem atualmente como modos de resistência às políticas neoliberais e aos retrocessos políticos remanicomializantes, demonstrando uma riqueza de formas de participação e circulação social e ampliação da cidadania fundamentais para repensar os processos de desinstitucionalização nas políticas de saúde mental no país.
