Introdução e objetivos: A mielopatia associada ao HTLV-1/paraparesia espástica
tropical (MAH/PET) é uma doença neurológica crônica e incapacitante, endêmica no
Brasil, que afeta uma população relativamente jovem. Múltiplos aspectos sobre
evolução da doença, perfil da incapacidade e opções terapêuticas permanecem obscuros
pela inexistência de uma ferramenta adequada de mensuração de saúde específica para
avaliação destes pacientes. As escalas utilizadas rotineiramente são incompletas ou de
difícil aplicação na prática clínica. Este estudo tem como objetivo principal a avaliação
do desempenho da escala EIPEC-2, que vem sendo utilizada informalmente há vários
anos em pacientes com MAH/PET, com boa aceitação entre os aplicadores.
Métodos: Oitenta e cinco pacientes com o diagnóstico de MAH/PET foram incluídos
no estudo. Foram aplicadas as escalas EIPEC-2, EIPEC-1, DSS, EDSS e Osame, assim
como os questionários de depressão de Beck e de qualidade de vida WHOQOL-bref por
um primeiro avaliador, e a EIPEC-2 por um segundo avaliador. Variáveis clínicas e
sociodemográficas também foram coletadas. Foi realizada análise de confiabilidade
inter-avaliadores da EIPEC-2, assim como de correlação dessa com as outras escalas e
os questionários de depressão e qualidade de vida. A aplicabilidade da EIPEC-2
também foi avaliada.
Resultados: A EIPEC-2 apresentou alta confiabilidade em geral, exceto pelos escores
de espasticidade. A escala foi também capaz de discriminar os diferentes graus de
incapacidade, com boa distribuição da amostra analisada. Houve alta correlação com as
outras escalas analisadas. As escalas EIPEC-2 e a EIPEC-1 apresentaram baixa, porém
significativa, correlação com as escalas de qualidade de vida e depressão, enquanto as
demais não se correlacionaram significativamente. A análise da aplicabilidade da escala
foi boa, com boa aceitação entre os aplicadores e baixo tempo de execução.
Discussão: A EIPEC-2, apesar do bom desempenho geral, ainda necessita de ajustes,
principalmente no que se refere ao escore de espasticidade. A EIPEC-1 apresentou
desempenho semelhante, porém tem pior aceitação entre os aplicadores.
Conclusões: A EIPEC-2 é de fácil aplicação, confiável na maioria de seus domínios, e
obteve desempenho superior às outras escalas habitualmente utilizadas para avaliação
de incapacidade na MAH/PET.

Introduction and objectives: HTLV-1 associated myelopathy/ tropical spastic
paraparesis (HAM/TSP) is a chronic and disabling neurological endemic disease in
Brazil, affecting a relatively young population. Multiple aspects of the disease,
disability profile and therapeutic options remain unclear due to the lack of a suitable
health measurement instrument to evaluate these patients. The scales used routinely are
incomplete or difficult to apply in clinical practice. This study aims to evaluate the
performance of EIPEC-2, which has been used informally for several years in patients
with HAM/TSP, with good acceptance among users.
Methods: Eighty-five patients diagnosed with HAM/TSP were included in the study.
The EIPEC-2, EIPEC-1, DSS, EDSS and Osame scales were applied, as well as Beck
depression inventory and WHOQOL-bref quality of life questionnaire by a first
interviewer, and EIPEC-2 by a second one. Clinical and sociodemographic variables
were also collected. Inter-rater reliability analysis of EIPEC-2 was carried out, in
addition to correlation analysis with the other scales and with the depression and the
quality of life questionnaires. The applicability of EIPEC-2 was also evaluated.
Results: In general, the EIPEC-2 showed a high reliability except for spasticity scores.
The scale adequately discriminated the different degrees of disability, with good
distribution of the analyzed sample. There was a high correlation with the other
investigated scales. EIPEC-2 and EIPEC-1 presented low, but significant, correlation
with the depression and quality of life scales, while the others did not show any
correlation. The analysis of the applicability of the scale showed good acceptance
among users and short application time.
Discussion: Although EIPEC-2 showed good overall performance, the scale still
requires adjustments, mostly with respect to the spasticity scores. EIPEC-1 showed
similar performance, but had worse acceptance among users.
Conclusion: The EIPEC-2 scale is easy to use, reliable in most of its areas, and showed
better performance to other commonly used scales for assessing disability in HAM/TSP.
