resistentes a carbapenêmicos (ERC) tornaram-se uma grande preocupação em termos de saúde pública mundial, tanto por sua elevada letalidade, quanto pelas limitadas opções terapêuticas para tratamento das mesmas. No Brasil, o primeiro relato de enterobactérias produtoras de carbapenemase foi feito em 2005, enquanto que a detecção de Klebsiella pneumoniae carbapenemase (KPC) aconteceu em 2009. A partir de então, outros relatos surgiram no país, embora para alguns estados brasileiros os dados ainda sejam escassos, como é o caso da Bahia. OBJETIVO: Descrever letalidade, características sócio-demográficas e clínicas de pacientes com ICS por enterobactérias em dois hospitais terciários de Salvador-BA, identificando as principais enterobactérias isoladas e seu perfil de sensibilidade antimicrobiana, além de determinar a taxa de infecção por ERC na população estudada, fatores de risco para infecções por ERC e para óbito, e descrever os principais regimes antimicrobianos utilizados para tratamento destas infecções. MÉTODOS: Trata-se de estudo de corte transversal em que foram incluídos casos de ICS por enterobactérias provenientes de dois hospitais terciários de Salvador, Bahia, no período de 01/04/2016 a 28/02/2018. Dados epidemiológicos foram coletados por meio de revisão de prontuários e os isolados bacterianos foram identificados por espectrometria de massa e pelo VITEK-2®. O perfil de susceptibilidade aos antimicrobianos foi realizado através de automação. RESULTADOS: No período do estudo, foram identificados 242 pacientes com ICS por enterobactérias, com um total de 255 isolados, sendo Escherichia coli e Klebsiella pneumoniae as mais frequentes. A letalidade geral das ICS foi de 40,9%, com K. pneumoniae e Proteus mirabilis apresentando os perfis de sensibilidade mais desfavoráveis. A taxa de resistência aos carbapenêmicos foi de 17,7%, sendo a letalidade para este grupo de 74,3%. Infecção por K. pneumoniae e P. mirabilis, internação em unidade de terapia intensiva e colonização prévia por gram-negativo resistente a carbapenêmicos foram fatores de risco independentes para infecções por ERC, enquanto que escore de Piit ≥ 4 e qSOFA ≥ 2 foram fatores de risco independentes para o óbito. Não houve homogeneidade na escolha dos esquemas antibióticos utilizados para tratamento das ICS por ERC. CONCLUSÕES: As ERC representaram 17,7% dos isolados do estudo. As ICS causadas por estas actérias apresentaram elevada letalidade,sendo a compreensão de sua epidemiologia fundamental para o estabelecimento de melhores estratégias terapêuticas e para adoção de medidas preventivas.
Abstract
Bloodstream infection (BSI) caused by carbapenem resistant
enterobacteria (CRE) has become a global public health concern because of its high lethality rate and scarce therapeutic options. In Brazil, the first report of an Enterobacteriaceae producing a carbapenemase came out in 2005, while the first detection of Klebsiella pneumonia carbapenemase (KPC) was published in 2009. Since then, new reports and studies emerged around the country, but they remain scarce in some states. OBJECTIVE: To describe lethality rate, epidemiological and clinical characteristics of patients with enterobacteria BSI ant to determinate carbapenem resistance rate between enterobacteria isolates, identifying major risk factors for CRE infection and mortality, and also to describe the most common therapeutic options used to treat these infections. METHODS: Between April 2016 and February 2018 we conducted a cross sectional study in two tertiary hospitals of patients with bloodstream infections caused by enterobacteria in which epidemiological
data were collected from medical records review. The bacterial isolates were identified by mass spectrometry and by VITEK-2®. Antimicrobial susceptibility testing was also performed by VITEK-2®. RESULTS: In the study period, 242 cases of bloodstream infection were identified, with a total of 255 enterobacteria isolates. Escherichia coli and Klebsiella pneumoniae were the most frequent enterobacteria isolated. Overall mortality rate was 40.9% and K. pneumoniae and Proteus mirabilis had the worst antimicrobial susceptibility profile. Carbapenem resistance was present in 17.7% of the isolates and for patients with BSI caused by CRE, mortality rate was 74.3%. K. pneumoniae e P. mirabilis infection, previous admission to intensive care unit and colonization by carbapenem-resistant gram-negative bacteria were independent risk factors for BSI caused by CRE. Pitt score ≥ 4 and qSOFA ≥ 2, in its turn, were independent risk factors for death. CONCLUSIONS: CRE represented 17.7% of the isolates and BSI caused by these bacteria had high mortality rate. Therefore, precise comprehension of its epidemiology is crucial for establishing better therapeutic and prevention strategies.
