O Brasil é campeão de cesáreas no mundo, atingindo 57,15% em 2014. A Organização
Mundial da Saúde – OMS indica que taxa de cesáreas acima de 15% não apresentam
benefícios. Estudos mostram que em geral, as taxas de cesáreas são maiores nas
macrorregiões e estados mais ricos, com maior Índice de Desenvolvimento Humano -
IDH e com maior percentual de população com acesso a planos de saúde. O objetivo geral
do estudo foi analisar as desigualdades regionais nas taxas de cesarianas nas regiões de
saúde do estado do Rio de Janeiro, no período de 2009 a 2017. A metodologia valeu-se
de uma abordagem quali-quantitativa. Foi realizada uma revisão da literatura para e
análise documental a partir dos documentos produzidos pelo Ministério da Saúde e pela
Secretaria de Estado de Saúde. Para caracterização das regiões de saúde do estado do Rio
de Janeiro foi construída uma matriz inspirada no modelo da Matriz de Dimensões da
Avaliação de Desempenho do Sistema de saúde do PROADESS (ICICT/FIOCRUZ).
Também foi realizado estudo de caso na Região de saúde Noroeste do estado do Rio de
Janeiro, que foi selecionada segundo o critério de menor taxa de partos normais (10%).
Os dados analisados para as regiões de saúde do ERJ evidenciaram altas taxas de cesáreas
no SUS e as desigualdades e diversidades regionais relacionadas à conformação da rede
assistencial e às condições socioeconômicas da população condicionam a possibilidade
de realização de partos vaginais ou cesarianas. Contradizendo as tendências da maior
parte dos estudos até aqui apresentados, a menor taxa de partos normais pertence à região
de saúde com menor riqueza, IDH, escolaridade e cobertura de planos de saúde
(Noroeste). As regiões de saúde do ERJ menos providas e menos diversificadas do ponto
de vista dos equipamentos, profissionais e dispositivos de saúde SUS tenderam a
representar menor possibilidade de realização de partos vaginais. As condições regionais
importam e, por vezes, podem se sobrepor às condições de raça e classe social. A Rede
Cegonha se constitui como importante política para indução da mudança de modelo de
parto e nascimento no ERJ. No entanto, seu foco está muito atrelado à investimento em
equipamentos e menos em profissionais e o modelo preconizado pela RC está disponível
em poucos serviços. Os resultados apontam para uma crescente mercantilização não
apenas da cesárea, mas também do parto normal.
Abstract
Brazil is the world's cesarean champion, reaching 57.15% in 2014. The World Health
Organization (WHO) indicates that cesarean rates above 15% do not present benefits.
Studies show that, in general, cesarean rates are higher in the macroregions and richer
states, with a higher Human Development Index (HDI) and with a higher percentage of
population with access to health insurance. The general objective of the study was to
analyze the regional inequalities in the rates of cesarean sections in the health regions of
the state of Rio de Janeiro, from 2009 to 2017. The methodology was based on a
qualitative-quantitative approach. A review of the literature for and documentary analysis
was done from the documents produced by the Ministry of Health and by the State
Department of Health. A matrix based on the Model of Dimension of the Performance
Evaluation of the Health System of PROADESS (ICICT / FIOCRUZ) was built to
characterize the health regions of the state of Rio de Janeiro. A case study was also
conducted in the Northwest Health Region of the state of Rio de Janeiro, which was
selected according to the criterion of a lower rate of normal births (10%). The data
analyzed for the health regions of the ERJ showed high rates of cesarean sections in the
SUS and regional inequalities and diversities related to the conformation of the health
care network and the socioeconomic conditions of the population condition the possibility
of performing vaginal or cesarean deliveries. Contrasting the tendencies of most of the
studies presented here, the lowest rate of normal births belongs to the region of health
with lower wealth, HDI, schooling and coverage of health plans (Northwest). The less
well-equipped and less diversified ERJ health regions from the point of view of SUS
equipment, professionals and healthcare devices tended to represent less possibility of
performing vaginal deliveries. Regional conditions matter and sometimes overlap with
conditions of race and social class. The Rede Cegonha constitutes an important policy to
induce the change of birth and birth model in the ERJ. However, its focus is very much
related to the investment in equipment and less in professionals and the model advocated
by RC is available in few services. The results point to a growing commercialization not
only of cesarean section, but also of normal birth.
