O Zika Vírus (ZIKV), teve recentemente uma rápida disseminação e foi vinculado à Síndrome de Guillain-Barré e ao nascimento de bebês com microcefalia, levando a Organização Mundial de Saúde (OMS) a declarar Emergência de Saúde Pública de importância Internacional. Diante deste cenário, ensaios destinados ao diagnóstico da doença foram disponibilizados no mercado nacional e internacional. O presente trabalho teve como objetivo realizar uma revisão integrativa da literatura científica, referente à qualidade dos kits utilizados no diagnóstico da infecção pelo ZIKV em suas diferentes metodologias, frente aos parâmetros de sensibilidade e especificidade. A busca foi realizada nas bases de dados Scielo, Scopus, PubMed e Google Acadêmico, e os artigos selecionados entre 2007 e 2017, em língua portuguesa e inglesa tendo como palavras-chave: “Zika virus”, “Diagnóstico”, “Kits”, “Teste Rápido”, “PCR”, “ELISA”, “Sensibilidade”, “Especificidade”, “Aedes”, “Flavivírus” e “Qualidade”. Os ensaios disponíveis para diagnóstico laboratorial de Zika são PCR em transcriptase reversa em tempo real (rRT-PCR), amplificação mediada por transcrição (TMA), detecção de anticorpos neutralizantes de ZIKV via teste de neutralização de redução de placa (PRNT), imuno-histoquímica, imunofluorescência, teste rápido (imunocromatográfico) e ensaio imunoenzimático (ELISA). Dos artigos selecionados, nem todos os ensaios foram abordados, apesar de já estarem disponíveis comercialmente, como teste rápido e imunofluorescência. Um total de sete artigos foram avaliados: quatro empregando o teste ELISA (abordando IgM; IgM/IgG; IgM, IgG e IgM/IgG), dois RT-PCR e um TMA. A sensibilidade dos kits ELISA variou de 23,7% a 88,2% para IgG; 20,7% a 58,8% para IgM; 39,5% a 100,0% para IgM/IgG. A especificidade foi de 95,2% a 100,0% para IgG; 95,7% a 100,0% para IgM; 92,5% a 99,8% para IgM/IgG. O limite de detecção para RT-PCR foi de 7 a 140 cópias/reação e 100% de especificidade. Para TMA foi observado 94,7% de sensibilidade e 94,8% de especificidade. Esta revisão mostrou valores aceitáveis ou até abaixo do limite de sensibilidade e especificidade dos kits analisados quando comparados aos parâmetros sugeridos pela OMS, o que pode levar a um diagnóstico incorreto colocando a saúde da população em risco. Vale ressaltar o baixo número de artigos que abordam os valores dos atributos de sensibilidade e especificidade em relação ao vasto número de artigos sobre a infecção pelo ZIKV.
Abstract
Zika virus (ZIKV) has recently spread rapidly throughout the world and has been linked to Guillain-Barré syndrome and the birth of infants with microcephaly, leading the World Health Organization (WHO) to declare Public Health Emergency of International Importance. In view of this scenario, tests destined to the diagnosis of the disease were made available in the national and international market. The present work aimed to carry out an integrative review of the scientific literature regarding the quality of the kits used in the diagnosis of ZIKV infection in its different methodologies, against sensitivity and specificity parameters. The search was carried out in the databases Scielo, Scopus, PubMed and Academic Google, and the articles selected between 2007 and 2017, in Portuguese and English, with the following keywords: "Zika virus", "Diagnosis", "Kits" PCR "," ELISA "," Sensitivity "," Specificity "," Aedes "," Flavivirus "or" Quality ". The available tests for laboratory diagnosis of Zika are real-time reverse transcriptase (rRT-PCR), transcription-mediated amplification (TMA), detection of neutralizing antibodies of ZIKV via plaque reduction neutralization test (PRNT), immuno-histochemistry, immunofluorescence, rapid test (immunochromatographic) and enzyme-linked immunosorbent assay (ELISA). Of the selected articles, not all trials were addressed, although they were already commercially available, such as rapid test and immunofluorescence. A total of seven articles were evaluated: four using the ELISA test (IgM; IgM / IgG; IgM, IgG and IgM / IgG), two RT-PCR and one TMA. The sensitivity of the ELISA kits ranged from 23.7% to 88.2% for IgG; 20.7% to 58.8% for IgM; 39.5% to 100.0% for IgM / IgG. The specificity was 95.2% to 100.0% for IgG; 95.7% to 100.0% for IgM; 92.5% to 99.8% for IgM / IgG. The detection limit for RT-PCR was 7 to 140 copies / reaction and 100% specificity. For TMA, 94.7% sensitivity and 94.8% specificity were observed. This review showed values acceptable or even below the sensitivity and specificity limit of the analyzed kits when compared to the parameters suggested by WHO, which may lead to an incorrect diagnosis placing the health of the population at risk. It is worth emphasizing the low number of articles that address the values of sensitivity and specificity attributes in relation to the vast number of articles on ZIKV infection.
