Esse estudo tem como objeto a participação social a partir da Política de Transplantes do estado do Rio de Janeiro e suas transformações históricas. A participação social sob a perspectiva do controle social foi observada com base nas experiências das Associações de doentes e pós-transplantados do estado. Buscou-se captar o reconhecimento e as participações das associações junto ao Conselho Estadual de Saúde do Rio de Janeiro (CES-RJ), recuperando para isso as contradições do seu processo de formação, que se relacionam ao processo de formação das políticas de saúde do país e em curso no Sistema Único de Saúde (SUS) no âmbito estadual. As Associações de doentes foram submetidas à entrevistas, além do confronto destas informações com os dados oriundos das Atas do CES-RJ, onde observa-se certo grau de mobilização e ocupação deste Conselho, na medida que as associações pressionam os serviços de transplante. As Associações de doentes estabelecem críticas ao CES-RJ, referindo o seu atravessamento por interesses corporativistas e extrapolam esta avaliação para o Programa Estadual de Transplantes do Rio de Janeiro, onde apontam para o estabelecimento de relações médico-hospitalocêntricas na organização e coordenação dos serviços, em detrimento dos usuários. Consideram ainda que o alargamento dos canais e espaços de diálogo com os doentes seria capaz de minimizar desigualdades de acesso à saúde, logo, o transplante. Refletir sobre os espaços de participação e seus mecanismos de controle social na saúde são fundamentais para a defesa do SUS frente as investidas para seu desmonte pelo capital.
Abstract
This study aims social participation based on the Rio de Janeiro´s Policy of Transplantation and its historical transformations. The social participation, under the perspective of Social Control, was observed based on the experiences of patient´s and transplanted associations from Rio de Janeiro. It sought to identify the recognition and the participation of associations in the Rio de Janeiro´s Health Council (CES-RJ) by recovering contradictions of their conformation process, which relates to health policies of the country formation and those in progress in the Unified Health System (SUS) at the state level. Information obtained from interviews conducted in the Associations was confronted with data from the CES-RJ meeting minutes. Thereafter, it was observed a certain degree of mobilization and occupation of this Council, as the Associations put pressure on the transplant services. It was also verified that Associations criticize the CES-RJ, referring to their crossing by corporatist interests and extrapolate these criticisms to the State Program of Transplants of Rio de Janeiro, where they point to the establishment of hospitalocentric relationships in the organization and coordination of services, to the detriment of users. They also believe that broadening the channels and spaces for dialogue with patients would be able to minimize inequalities in access to health. Based on this study, it can be inferred that reflecting on participation spaces and their mechanisms of social control in health is fundamental for the defense of the SUS against those invested in its dismantling by the capital.
