Realizou-se estudo prospectivo com 23 pacientes, sendo 14 crianças e 9 adultos com leishmaniose visceral (LV), em São Luiz no período de julho de 2000 a agosto de 2002, com 0 objetivo de avaliar os aspectos clinico - laboratoriais e marcadores precoces de eficácia terapêutica na LV, em crianças e adultos, antes e após 0 tratamento. Utilizou-se uma ficha-protocolo para coleta dos dados clínico-Iaboratoriais. Realizou-¬se avaliação imunol6gica através do teste de enzyme linked immunosorbent assay (ELISA) das concentrações de imunoglobulinas (IgG, IgG1, IgG2, IgG3, IgG4 e IgE), citocinas (IFN-γ, TNFα e IL-10) em sobrenadantes de cédulas mononucleares de sangue periférico (CMSP) estimulados com antígeno de Leishmania e citocinas plasmáticas (IFN- γ, TNF-α, IL-10 e IL-12p40) no dia zero, no termino do tratamento (30 dias) e após 0 tratamento (120 e 210 dias), bem como realização da intradermorreação de Montenegro (IDRM) e blastogênese com antígeno de Leishmania. A duração da doença, linfoadenopatia e sangramento foram estatisticamente significantes no grupo de adultos, hepatomegalia e palidez cutâneo-mucosa, significantes no grupo de crianças. Observou-se diferença significante entre os dois grupos no percentual de variação (valor final menos valor inicial, dividido pelo valor inicial) dos segmentados, linf6citos e velocidade de hemossedimentação, bem como as concentrações de IgG, IgG1, IgG3 e IgG4. A queda de IgG e IgG1 foram mais acentuadas aos 120 dias, mas permaneceram detectáveis nos tempos avaliados. O aumento das concentrações de IgG parece ser devido a ativacão policlonal de células B e a diferença entre os dois grupos, seja porque as concentrações séricas de IgG variam com a idade. A maioria dos pacientes (91 por cento) apresentou concentrações detectáveis de IgE antes do tratamento, mantendo-se inalterados nos tempos avaliados. Observou-se resposta linfoproliferativa em 14 por cento dos indivíduos no termino da terapêutica, e em 48 por cento aos 120 dias após o tratamento. Setenta e sete por cento dos pacientes converteram a IDRM após seis meses de tratamento. A explicação para este achado seria a população de células participantes da IDRM diferente da envolvida na blastogênese. Detectou-se, na maioria dos indivíduos, elevadas concentrações plasmáticas de IFN-γ (90 por cento), IL-10 (90 por cento) e IL-12p40 (86 por cento) no dia zero, com queda acentuada de IFN-γ (43 por cento) ao termino do tratamento. Por outro lado, em sobrenadante de CMSP as concentrações de IL-10 encontravam-se elevadas e as de IFN-γ não detectáveis, no dia zero. Em seguida, as concentrações de IL-10 dec1inavam e as de IFN-γ aumentavam, à medida que avançava 0 tratamento. O TNF-α foi detectado somente em cultura de CMSP ap6s 0 tratamento. Em conseqüência de uma resposta terapêutica favorável foi observada redução precoce da produção de citocinas plasmáticas tanto intlamat6rias como supressoras. A depressão da resposta imune celular presente nos pacientes de LV pode ser devida à predominância da atividade biol6gica de IL-10 e não à ausência de INF- γ. As concentrações plasmáticas de citocina do tipo Th1 (IFN-γ) no dia zero e diminuição no termino do tratamento, podem constituir-se num parâmetro de resposta à terapêutica na leishmaniose visceral humana.

A prospective study was conducted on 23 patients with visceral leishmaniasis (VL), including 14 children and 9 adults, in São Luis, Maranhão, during the period from July 2000 to August 2002, in order to determine clinical and laboratory aspects and early markers of therapeutic efficacy in children and adults with VL before and after treatment. A protocol chart was used for collection of the following data clinical-laboratory. For immunological assessment, immunoglobulins (IgG, IgGl, IgG2, IgG3, IgG4 and IgE), plasma cytokines (IFN-y, TNF-a, IL-10 and IL-12p40) and Leishmania in antigen-stimulated peripheral blood mononuclear cell (PBMC) cytokines (IFN-y, TNF-a and IL-10) were determined by enzyme-linked immunosorbent assay (ELISA) on days zero, 30 (end of treatment), 120 and 210 (after treatment). In addition, the Montenegro skin test (MST) and blastogenesis with anti- Leishnmnia antigen were also performed. Disease duration, adenomegaly and bleeding were significant in the adult group, while hepatomegaly and mucocutaneous paleness were significant in the children group. A significant difference between the two groups was found in the percent variation (final values minus initial value divided by initial value) of segmented leukocytes, lymphocytes and erythrocyte sedimentation rate, as well as in the levels of IgG, IgGl, IgG3 and IgG4. In IgG and IgGl decreasing was more evident at 120 days, but both remained detectable at the other time points analyzed. The increase in IgG levels seems to be the result of polyclonal B cell activation and the difference observed between the two groups can be due to the fact that serie IgG concentrations change with age. Most patients (91%) had detectable IgE levels before treatment, which remained unchanged at the time points studied. A lymphoproliferative response was observed in 41% of patients at the end of treatment, and 48% in day 120 after treatment. The conversion of MST was observed in 77% of patients after 6 months of treatment, what could be explained by differences between the cellular population involved in MST and blastogenesis. in the most of patients was detected great plasmic levels of IFN-y (90%), IL-10 (90%) and IL-12p40 (86%) in the day zero, with a significant decreasing of IFN-y (43%) in the end of treatment. On the other hand, PBMC levels of IL-10 were high on day zero, while IFN-y was undetectable. Subsequently, IL-10 levels declined and IFN-y increased with advanced treatment. TNF-a was only detected in PBMC cultures after treatment. The results indicate that a favorable therapeutic response leads to an early reduction in the production of both inflammatory and suppressive cytokines in plasma. The cellular immune depression observed in patients with VL might be due to a predominance of the biological activity of IL-10 and not to the lack of IFN-y. The high plasma levels of both Thl type (IFN-y, IL-12p40) and Th2 type (IL-10) cytokines on day zero and their decreasing at the end of treatment may represent good parameter to indicate the therapeutical efficacy in human VL.
