A introdução dos antiretrovirais (ARV) tem possibilitando aumento na sobrevida e qualidade de vida de pacientes infectados pelo HIV. Seus para-efeitos estão associados a mudanças somáticas e metabólicas, relacionadas à maior risco de doenças arteriais coronarianas e periféricas. O estudo da função endotelial se presta como marcador pré-clínico de doença cardiovascular. Avaliação da função endotelial de indivíduos portadores do HIV. Estudo transversal com grupo de comparação em que foram avaliados 69 pacientes: 44 HIV, idade 37,9 ± 8,3 (2 1-56) anos ,e 25 controles normais, idade 41,7 ± 12,3 (20 -56) anos. Dentre os pacientes HP 11 não faziam uso de ARV. Realizou-se estudo de ultrasom vascular através da análise da vasodilatação observaram-se por fluxo (VMF), expressa por mudança no diâmetro vascular pós-estímulo. Obseraram-se mudanças percentuais das medidas de dilatação em relação à fase basal. Nos pacientes portadores do HP!, utilizou-se dinitrato de isossorbida para avaliação da resposta vascular da musculatura lisa (avaliação endotélio independente). Medidas de diâmetros foram comparadas nas diversas fases através do teste t Student, e nas variáveis que não apresentaram uma distribuição normal utilizou-se o teste Mann-Whitney, com valor de pSO,005. Em análise de regressão logística, foi utilizada a VMF como variável dependente. Os pacientes portadores do 111V tiveram menor reposta de VMF que o grupo controle (10,9 ±7,5 e 23,3 ± 6,1% respectivamente, p=0. Os 33 pacientes que utilizavam a terapia ARV apresentaram menor VMF ( ± 6,9) em relação ao grupo HIV virgem de terapia (19,26 ± 4,8%) p=O,OO. Não se observou diferença de VMF, fase endotélio independente, com relação ao uso ou não de terapia ARY, (13,4 ± 6,6 e 13,0 ± 6,9%, respectivamente) p=O,89. O grupo HIV sem uso de ARV e grupo controle apresentaram valor de VMF (19,26 ± 4,8 e 23,3 ±4,8%, respectivamente) próximo ao nível de significância (p=O,O6). O uso de Inibidores de Protease no esquema terapêutico ARY não apresentou diferença de resposta VMF nos grupos estudados. A avaliação laboratorial incluiu CD4, Carga Viral, perfil lipídico, glicemia, ácido úrico, transaminase glutâmica pirúvica e oxalacética. A terapia ARV foi o fator preditor de interferência na análise associado à VMF. Os demais fatores que foram associados à VMF na análise univariada não tiveram influência quando testados na análise multivariada. A presença de disfunção endotelial em pacientes portadores do HIV em uso de ARV sugere tratar-se de população especial, que deve ser orientada quanto aos fatores de risco de doença cardiovascular.

Antiretroviral (ARV) therapy in HIV-infected patients has been associated with dyslipidaemia and its risk factor for cardiovascular disease is unclear. Endothelial dysfunction is an early phase of atherogenesis that may be assessed non-invasively with ultrasonography. Aim: To evaluate arterial endothelial function in HIV-infected subjects Design: Observational cross-sectional prospective study. Metliods: We studied 44 HIV- infected individuals (33 ARV-treated and 11 najfve) and 25 healthy controls. Brachial artery endothelial function was studied using high-resolution ultrasound. This non- invasive technique measured post-ischemic flow-mediated dilatation (FMD), and the endothelium-independent vasodilator response to glyceryl trinitrate (GTN). Student’s t test and Mann-Whitney test, was used as appropriate to compare the mean of the continuous variables when appropriated. Multiple regression analysis was carried out using VMF as the dependent variable, p values less than 0.05 were considered significant. Results Analyses of post-ischemic flow-mediated dilatation of the brachial artery (FMD) was significantly lower for the ARV therapy-experienced patients compared to the naive and those also were significantly different from the controls (ARV experienced 8,2 ±6.0 vs. 19.3 ±4.8 vs. 23,3 (±6,1) respectively p< 0.0001. The cholesterol, triglyceride, uric acid and ALT liver enzymes levels were significantly higher in the ARV-treated group compared to naive and controls p< 0.028, however, multilinear linear logistic regression analysis revealed an association of the dependent variable endothelial dysfunction with only ARV therapy (p=0,0001). There was no association with dyslipidaemia. Conclusions: endothelial dysfunction was associated with HIV -infected patients and ARV therapy, and this population should have be a carefully follow up for cardiovascular disease.
