ESTUDO ANÁTOMO-PATOLÓGICO DE NOVENTA E CINCO FÍGADOS DE PACIENTES TRANSPLANTADOS NO ESTADO DA BAHIA ENTRE 2001 E 2008

o primeiro transplante hepático feito na Bahia ocorreu em 2001 e foi realizado pela ,equipe da Real Sociedade Portuguesa de Beneficência Dezesseis de Setembro (Hospital Português da Bahia). Entre 2001 e 2008, cento e vinte e cinco transplantes de fígado foram feitos no Estado, sendo que 95 deles foram feitos pela equipe do Hospital Português (76% de todos os transplantes realizados). O presente trabalho mostrou o, estudo sistemático de 95 fígados provenientes de pacientes transplantados por esta equipe. Os aspectos anátomo-patológicos dos fígados explantados foram avaliados e foi feito um estudo mais aprofundado dos casos de pacientes transplantados sem doença de base definida pré-transplante. Nesta série, 81,91% dos transplantados foram do sexo masculino e 18,08% do sexo feminino. A média de idade dos transplantados foi de 50,65 anos. Em relação aos diagnósticos clínicos pré-transplante, 34,73% tinham infecçcão pelo VHC, 20% tinham doença alcoólica do fígado, 16,84% não tinham diagnóstico de primário de doença hepática pré-transplante definido, 5,26% tinham infecção crônica pelo VHB, dentre outros. O estudo anátomo-patológico dos explantes mostrou cirrose em 84 casos. Em 10 fígados não havia cirrose (10,63%) (um explante, consistiu em um enxerto não funcionante, que foi excluído da análise). Carcinoma hepatocelular foi diagnosticado em 22 fígados (23,40% da amostra), esteato-hepatite em 34 explantes (36,17% da amostra). Em 75% dos casos sem diagnóstico da hepatopatia definido pré-transplante, o estudo anátomo-patológico foi elucidativo, mostrando esteato-hepatite, cirrose de padrão biliar, doenças relacionadas com a veia porta e hepatite auto-imune dentre os principais diagnósticos primários de doença hepática. Em cinco casos de pacientes que tinham quadro clínico de insuficiência hepática e hipertensão porta antes do transplante não foi visto cirrose. Estes casos mostraram alterações relacionadas à veia porta, sendo diagnosticado em três deles esclerose hepatoportal e em dois deles fibrose de Symmers. Aspectos anátomo-patológicos detalhados e correlações clínicas destes casos foram apresentados e discutidos. Não há na literatura brasileira relatos de pacientes transplantados com este perfil e na literatura internacional os relatos são escassos.
Abstract
The first liver transplant was done in Bahia in 2001. It was performed by the Real Sociedade Portuguesa de Beneficência Dezesseis de Setembro (Portuguese Hospital of Bahia). One hundred and twenty five liver transplants were done in Bahia between 2001 and 2008, 95 of which were done by the Portuguese Hospital’s team (76% of all transplants done). The present work described the pathological findings of these 95 liver explants and analyzed in more detail cases of patients transplanted without a primary liver disease diagnosed before the transplant. In this series of cases, 81,91% of patients were male and 18,08% were female. The median age of the transplanted patients was 50,65. Clinical diagnosis before transplant included VHC infection in 34,73%, alcoholic liver disease in 20%, undefined liver disease before transplant in 16,84%, VHB infection in 5,26%, among others. Pathological study revealed cirrhosis in 84 cases and absence of cirrhosis in 10 explants (10,63%) (one explant consisted of a non-function graft, that was excluded from the analysis). Primary liver carcinoma was diagnosed in 22 explants (23,40%), steato-hepatitis was present in 34 explants (36,17%). Pathological study was conclusive regarding liver disease in 75% of cases without a hepatic disease known before transplant. Five cases, from patients with liver failure and portal hypertension before transplant whose explants did not show cirrhosis were deeper discussed; three of them were diagnosed with hepatoportal sclerosis and two were diagnosed with Symmers fibrosis. Pathological aspects with clinical data of these cases were discussed. In Brazilian literature there are no reports of liver-transplanted patients with this profile and there are few international reports as well.
