Diferentes aspectos clínico-epidemiológicos e patológicos têm sido descritos em neoplasias ocorrendo em pacientes HIV positivos (HIV+). Linfomas fazem parte deste grupo, sendo o Linfoma Não-Hodgkin (LNH) a segunda neoplasia mais comum em pacientes com AIDS. Porém, tem sido observada uma grande variação geográfica na frequencia de LNH em pacientes HIV -positivos, bem como na distribuição dos diferentes subtipos histológicos. Desde que pouco se sabe sobre LNH em pacientes HIV+ no nosso meio, realizamos este trabalho para: a) classificar de acordo com a OMS os LNH ocorrendo nestes pacientes, comparando com LNH em pacientes HIV-negativos (HIV-) em Salvador-Bahia; b) investigar associação com infecção pelo vírus Epistein-Barr (EBV) no tecido neoplásico destes pacientes. Estudo caso-controle retrospectivo. Foram levantados dados clínicos e epidemiológicos de casos de LNH diagnosticados em Salvador entre Janeiro de 1998 e Dezembro de 2002. Para tanto foram revisados os prontuários dos pacientes com LNH e/ou realizadas entrevistas com médicos envolvidos no atendimento em serviços de referência no atendimento de LNH e/ou AIDS em Salvador. Foram obtidos 17 casos e 30 controles. Os LNH foram submetidos a revisão histológica, imunoistoquímica e classificados segundo a OMS-2001. A infecção pelo EBV foi detectada por técnicas de imunoistoquímica e hibridização in situ. A média de idade dos LNH nos pacientes HIV + foi de 41 (+/-8,5) anos, inferior ao do grupo controle (p=O,OI). O acometimento primário de sítios extra-nodais foi mais freqüente nos casos (12/17) que nos controles (10/30) (p=0,004). Os LNH em pacientes HIV+ foram exclusivamente de fenótipo B e de alto grau de malignidade, enquanto que no grupo controle houve linfomas com fenótipo B e T e graus histológicos variáveis. O percentual de casos positivos para EBV foi significativamente maior nos casos que nos controles (p =0,002). Os LNH em pacientes HIV + têm aspectos clínicos e histopatológicos distintos dos descritos na população geral: apresentam alto grau de malignidade, têm tendência à manifestação primária em sítios extra-nodais e exibem uma alta associação com a infecção pelo EBV. É possível que o EBV desempenhe um papel importante no desenvolvimento de alguns dos subtipos de LNH em pacientes HIV +. Porém, há necessidade de estudo sistemático prospecti vo para melhor caracterização destes linfomas.

Distinct clinical-epidemiologic and pathologic aspects have been described in neoplastic diseases occurring in HIV-positive (HIV+) patients. Lmphomas are part of this group, and Non-Hodgkin "s Lymphoma (NHL) is the second most common cancer in AIDS. However, the frequency of LNH may be highly variable in different geographic areas, not only in HIV+ patients but also in immunocompetent patients. [OBJECTIVES] As there is little information about lymphomas in HIV+ patients in Salvador, Brazil, we proposed to study and classify according to OMS, LNH in HIV+ patients and compare their characteristic with lymphomas occurring in HIV-negative (HIV-) patients in Salvador-Bahia-Brazil. We also investigated the presence of the Epstein-Barr virus (EBV) infection in tumor tissue of these patients. [MATERIAL AND METHODS] A retrospective case-control trial. We revised the files of NHL patients and/or interviewed medical doctors involved in the treatment of AIDS and NHL in several clinics and hospitals in Salvador. We studied 17 HIV+ NHL (cases) and 30 HIV- NHL (controls). Paraffin sections of neoplastic tissue were analyzed by immunohistochemistry and classified according the OMS-2001. The detection of EBV infection was performed using in situ hybridization and immuohistochemistry. [RESULTS]. The LNH in HIV+ patients occurred in younger people when compared with the control group (p=0, 01). The primary involvment of extra-nodal sites was more frequent among HIV+ patients than in HIV- patients (p=0,004). NHL occurring in HIV+ patients were high grade lymphomas exclusively of B-cell imunophenotype. In the HIV- group there were variable histological grade and imunophenotype (T, B or null). The frequency of EBV infection in NHL was significantly higher in the HFV+ lymphomas (p=0,002). [CONCLUSIONS] NHL in HIV+ pacients has distinct histological and clinical aspects. These LNH are of high grade of malignance, have a tendency of primary extra-nodal presentation and a high association with EBV infection. EBV infection may be related to the development of some subtypes of LNH by HIV+ patients. However, it is necessary to study a larger series of cases for a better characterization of the NHL in the HIV-I- patients.
