LINFOMAS NÃO HODGKIN COM MANIFESTAÇÃO EXTRANODAL AO DIAGNÓSTICO EM SALVADOR - BAHIA: ASPECTOS CLÍNICOS E CLASSIFICAÇÃO HISTOPATOLÓGICA SEGUNDO A OMS - 2001

Linfomas não-Hodgkin (LNH) extranodais representam mais 20% de todos os linfomas e apresentam atualmente um aumento da taxa de incidência maior que LNH nodais. O trato gastrointestinal é o principal sítio acometido, seguido por cabeça e pescoço e pele. O sítio principal de acometimento pode variar conforme a região geográfica, assim como o subtipo histológico. Classificar, segundo a OMS-2001, os LNH ranodais ocorridos em Salvador-Bahia; observar o principal sítio extranodal acometido, bem como os principais subtipos histológicos; estudar as principais características clínicas e laboratoriais dos LNH extranodais em nosso meio. Estudo descritivo, retrospectivo, realizado no período de janeiro de 1999 a julho de 2001, através do levantamento de dados prontuários dos pacientes do Hospital Aristides Maltez, em Salvador-Bahia. Foram revistos 145 casos de LNH sendo que 44 casos eram de LNH com manifestação extranodal ao diagnóstico. Os casos com manifestação extranodal foram submetidos à revisão histológica, imuno-histoquímica e classificados segundo a OMS-2001. Foram excluídos casos com manifestação extranodal na recidiva, casos de mieloma múltiplo/plasmocitoma e 4 casos sem informações suficientes em prontuário. A freqüência de LNHs extranodais foi de 30,3%. A média de idade foi de 55,6 anos (variando de 17 a anos) e a relação homem/mulher foi de 1:1. A maioria dos pacientes apresentavam estadio avançado (111 ou IV), sintomas S, LDH normal, ECOG tre O e 1 e IPI entre O e 2. Nove pacientes estão vivos e em remissão Impleta (22,5%). O principal sítio extranodal acometido foram as tonsilas (11 casos), seguidas pela cavidade oral (9 casos), pele (9 casos) e estômago (5 150S), dentre outros. Em 7 pacientes havia dois sítios extranodais acometidos. 5ubtipo histológico mais freqüente foi o linfoma difuso de grandes células B (LDGCB), em 27 casos (71,1%). Diferentemente da maioria da literatura, encontramos as tonsilas como o mais freqüente sítio extranodal de acometimento dos LNH. O LDGCB foi o subtipo histológico mais freqüente.
Abstract
Extranodal non-Hodgkin’s lymplionna (NHL) represents more than 20% of all lymphomas and presently the incidence rate is higher than nodal lymphomas. The gastrointestinal tract is the main involved site, followed by the head and neck and the skin. The main involved site may vary according to the geographic region, as well as the histological subtype. [OBJECTIVES] Classify, according to the WHO-2001 the extranodal NHL that occurred in Salvador-Bahia; observe the main extranodal involved site, as well as the main histological subtypes; study the main clinical and laboratorial characteristics of the extranodal NHL in our city. [PATIENTS AND METHODS] Descriptive and retrospective study, which was carried out from January 1999 to July 2001, through collection of the data from patient’s charts from Aristides Maltez Hospital, in Salvador-Bahia. A hundred and forty five cases of NHL have been reviewed, 44 of which were NHL with extranodal manifestation at diagnosis. These cases were submitted to histological and immunohistochemistry review, and classified according to the WHO-2001. Cases in which extranodal manifestation occurred at relapse and cases of multiple myeloma / plasmacytoma were excluded, as well as 4 cases without sufficient information’s in their charts. [RESULTS] The frequency of extranodal NHL was 30.3%. The average age was 55.6 years (range 17 to 86 years-old), and the male/female ratio was 1:1. The majority of the patients showed advanced stages (III or IV), B symptoms, normal LDH, ECOG between 0 and 1, and IPI between 0 and 2. Nine patients are still alive in complete remission (22.5%). The main extranodal involved site were tonsils (11 cases), followed by oral cavity (9 cases), skin (9 cases), and stomach (5 cases), among others. Seven patients had two extranodal involved sites. The main histological subtype was diffuse large B-cell lymphoma (DLBCL) in 27 cases (71.1%). [CONCLUSIONS] Differently from most other studies, the tonsils were the most frequently involved site in NHL in Salvador-Bahia. DLBCL was the most frequent histological subtype.
