O Brasil juntamente com outros 21 países são responsáveis por 80% do número absoluto de casos da doença no mundo. Entre as unidades da federação, o Rio de Janeiro apresenta a segunda maior incidência e o maior coeficiente de mortalidade do país.Objetivo:Caracterizar a situação epidemiológica da Tuberculose Drogarresistente(TBDR)no estado do Rio de Janeiro a partir do relacionamento das bases de dados do GAL e do SITETB no período de 2011 a 2015.Métodos:Estudo epidemiológico observacional, de coorte retrospectiva,realizado através da análise das bases de dados do GAL e do SITETB do estado do Rio de Janeiro no período de2011 a2015.Foirealizada a análise descritiva da ocorrência da TBDR na área de abrangência do estudo. Na análisefoiutilizado o teste de qui-quadrado de Pearson (χ2) ou teste exato de Fisher para variáveis categóricas.Foram realizadasas estimativas de taxas de incidência, proporção dos tipos de encerramento a descrição epidemiológica e comparativa e dos casosde TBDR. Além disso, foirealizada avaliação da subnotificação através do método de relacionamento de bases de dados.Por fim, foi realizada descrição dos casos subnotificados em comparativo com os casos notificados.Os dados foramestruturados e organizados em planilhas Microsoft Excel 2010. Para as análises estatísticas e para o relacionamento das bases de dados foiusado o software STATA 13.0.Resultados:Foram estudados 1067 casos novos e 370 casos de retratamento, a comparação mostrou que os grupospossuem características diferentes estatisticamente significativas. Amaior proporção de casos foi registrada na região metropolitana I.O sexo masculino foi o mais acometido, principalmente entre os casos de retratamento. A forma pulmonar bilateral cavitáriafoi mais expressiva nos casos de retratamento. Em contrapartida a forma unilateral cavitária esteve mais presente entre os casos novos. A resistência adquirida foi oprincipal tipo de resistência(74,1%). A Tuberculose multirresistente (TBMR)predominou amplamente sobre os demais tipos, 19,1% dos casos de retratamento foram tuberculose extensivamente resistente (TBXDR).Quanto ao encerramento, 66,4% dos casos novos tiveram desfecho favorável enquanto apenas 32,7% entre os casos prevalentes. Na análise dasubnotificação foram evidenciados casos em todos os anos estudados, atingindo até 39% dos casos diagnosticados em 2014, a monorresistência à Isoniazida e a TBMR foram os mais prevalentes. Conclusões: Acredita-se que o relacionamento entre os banco de dados no SITETB e do GAL, pode ser um instrumento auxiliar no vigilância da resistência no país, a fim de se conhecer a situação epidemiológica mais precisa para tomada de decisões baseadas em evidências.
Abstract
Brazil together with 21 other countries are responsible for 80% of the absolute number of cases of the disease in the world. Among the units of the federation, Rio de Janeiro has the second highest incidence and the highest mortality rate in the country. Objective: To characterize the epidemiological situation of Drug Resistant Tuberculosis (TBDR) in the stateof Rio de Janeiro from the relationship of the GALand SITETB databases in the period from 2011 to 2015. Methods:Observational epidemiological study of a retrospective cohort conducted through Analysis of the GALand SITETB databases from the state of Rio de Janeiro in the period from 2011 to 2015. A descriptive analysis of the occurrence of TBDR was carried out in the area covered by the study. Pearson's chi-square test (χ2) or Fisher's exact test for categorical variables were used in the analysis. Estimates of incidence rates,proportion of types of closure, epidemiological and comparative description, and cases of TBDR were performed. In addition, evaluation of underreporting was performed through the database relationship method. Finally, a description of underreported cases was carried out in comparison with reported cases. The data were structured and organized into Microsoft Excel 2010 spreadsheets. Statistical analyzes and database relationships were used with STATA 13.0 software. Results:A total of 1067 new cases and 370cases of retreatment were studied. The comparison showed that the groups have different statistical characteristics. The highest proportion of cases was recorded in the metropolitan region I. The male sex was the most affected, especially among the cases of retreatment. The bilateral cavitary pulmonary form was more expressive in cases of retreatment. In contrast, the unilateral cavitary form was more present among the new cases. The acquired resistance was the main type of resistance (74.1%). Multidrug-resistant tuberculosis (MDR-TB) predominated widely over the other types, 19.1% of the cases of retreatment were extensively drug-resistant(XDR-TB). As for the closure, 66.4% of the new cases had a favorable outcome, while only 32.7% among the prevalent cases. In the underreporting analysis, cases were detected in all the studied years, reaching up to 39% of the cases diagnosed in 2014, monoresistance to Isoniazid and MDR-TB were the most prevalent. Conclusions: It is believed that the relationship between the databases in the SITETB and the GALcan be an auxiliary tool in the surveillance of resistance in the country, in order to know the most precise epidemiological situation for evidence-based making decision
