GLOMERULONEFRITE ASSOCIADA À ESQUISTOSSOMOSE HEPATOESPLÊNICA. (ESTUDO COMPARATIVO DOS TIPOS HISTOLÓGICOS EM CASOS COM E SEM HEPATOESPLENOMEGALIA ESQUISTOSSOMÓTICA)

O trabalho se refere a um estudo histopatologico compa rativo dos rins de 36 pacientes com esquistossomose hepatoes- plênica e glomerulonefrite crónica, e dos rins de um grupo con trole, da mesma região, formado por 36 pacientes com glomerulo nefrite, mas sem esquistossomose hepatoesplênica.
Os resultados revelarami que os tipos histológicos de glomerulonefrite mais encontrados em pacientes com esquistosso mose hepatoesplenica foram: glomerulonefrite mesangiocapilar (33,2%), glomerulonefrite proliferativa mesangial (25%) seguida de esclerose glomerular focal (16,7%) e de glomerulonefrite esclerosante (8,3%). No grupo controle, as glomerulopatias mais observadas foram: glomerulonefrite proliferativa endocapi- lar (27,8%), glomerulonefrite mesangiocapilar (19,4%), glomeru lonefrite proliferativa mesangial (13,9%), vindo a seguir a glomerulonefrite esclerosante (11,1%) e a esclerose glomerular focal (8,3%). Estes dados foram analisados estatisticamente através do teste de qui quadrado, (p<0,05), concluindo-se que não existem diferenças estatisticamente significantes entre os tipos histológicos de glomerulonefrite encontrados em ambos os grupos. Exceção foi feita com a glomerulonefrite proliferativa endocapilar, que foi estatisticamente mais prevalente no grupo controle.
Os dados agora obtidos estão em desacordo com aqueles da literatura e demonstram que, na mesma região (Salvador- Bahia) não parece haver influência da infecção esquistossomótica no predomínio dos tipos histológicos de glomerulonefrite mais encontrados.
Contudo, verificou-se que, o conjunto formado por glomerulonef rite mesangio-capilar e glomerulonefrite proliferativa mesangial foi estatisticamente mais prevalente no grupo dos pacientes com esquistossomose hepatoesplênica. Portanto, pare-
ce existir um envolvimento predominante e peculiar do mesangio em glomerulonefrites que incidem em portadores de hepatoesple- nomegalia esquistossomótica. Este dado está de acordo com alguns relatos da literatura os quais evidenciam que, frequentemente, pode haver comprometimento mesangial quando o rim é envolvido no curso de algumas doenças infecciosas e parasitárias, como é o caso da malaria causada pelo Plasmodium malariae e possivelmente também na leishmaniose visceral.

This work is concerned with a comparative histopathologic study of the kidneys from 36 subjects with chronic glomerulonephritis associated with hepatosplenic schistosomiasis and from 36 other individuals, from the sama geographical region,also with chronic glomerulonephritis, but without hepatosplenic schistosomiasis. The objective was to see whether there was a peculiar or predominant type or types of glomerulonephritis associated with hepatosplenic schistosomiasis.
Results revealed that the histologic types of glomerulonephritis associated with hepatosplenic schistosomiasis were: mesangio-capillary glomerulonephritis (33.2%), mesangial proliferative glomerulonephritis (25.0%), focal glomerular sclerosis (16.7%) and sclerosing glomerulonephritis (8.3%). In the control group, without hepatosplenic schistosomiasis, the following types were found: endocapillary proliferative glomerulonephritis (27.8%),mesangio- capillary glomerulonephritis (19.4%), mesangial proliferative glomerulonephritis (13.9%), sclerosing glomerulonephritis (11.1%) and focal glomerular sclerosis (8.3%).
When submitted to statistical analysis, considering p<0.05 in a chi-square test, these results failed to be significant, with one single exception: endocapillary glomerulonephritis was statistically more frequent in the control group.
These overall results did not substancíate the previous assumption that mesangio-capillary glomerulonephritis is specifically related to hepatosplenic schistosomiasis in
endemic areas.
However, if the involvement of the mesagium is considered separately, by adding the mesangio-capillary and

the mesangial proliferative types, it is a predominant finding associated with hepatosplenic schistosomiasis (p<0.05).
Therefore, it seems that a predominant mesangial involvement occurs in glomerulonephritis associated with hepatosplenic schistosomiasis, likewise in other parasitic diseases, such as malaria and probably visceral leishmaniasis.
