ANÁLISE CUSTO-EFETIVIDADE ATRAVÉS DO MÉTODO DE ANÁLISE ENVOLTÓRIA DE DADOS EM ENSAIO CLÍNICO DE TRATAMENTO COM DOSE PADRÃO E ALTERNATIVA DE ANTIMONIATO DE MEGLUMINA EM PACIENTES COM LEISHMANIOSE TEGUMENTAR AMERICANA NA FORMA CUTÂNEA

A leishmaniose cutânea constitui um dos principais problemas de saúde pública no Brasil. O medicamento de primeira escolha para seu tratamento é o antimoniato de meglumina, que apresenta, entretanto, considerável toxicidade sistêmica. Tratamento com doses altas (20mg do antimônio pentavalente - Sb5+- por kilograma de peso por dia por 20 dias) e doses baixas (5mg Sb5+/kg/dia por 30 dias) foram comparados anteriormente, porém sem chegar a resultados conclusivos. Um ensaio clínico de equivalência entre tratamento com dose alta e com dose baixa encontra-se em curso no Laboratório de Vigilância em Leishmanioses do Instituto de Pesquisa Clínica Evandro Chagas, Fundação Oswaldo Cruz, Rio de Janeiro, Brasil. Foi realizado um estudo de custo-efetividade aninhado neste ensaio clínico com os primeiros 59 pacientes com leishmaniose cutânea, com a utilização do modelo de Análise Envoltória de Dados (DEA) Buscou-se verificar, através do cálculo das médias dos escoressíntese de recursos (inputs) aplicados e resultados (outputs) obtidos com os pacientes submetidos aos diferentes tratamentos, qual das intervenções do ensaio pertence à fronteira de eficiência, e se alguma intervenção se revela como ineficiente. Verificamos que não houve diferença entre as intervenções do ensaio, segundo a comparação das médias dos escoressíntese pelo modelo DEA. Entretanto, pela comparação dos pares relativos a cada paciente com seus referenciais (benchmarks) na fronteira de eficiência, observamos que houve mais pacientes do grupo de tratamento com dose baixa que pacientes do grupo de dose alta na fronteira de eficiência. Portanto, o grupo de tratamento com dose baixa se mostrou mais eficiente que o grupo com dose alta. Estudos adicionais com maior número de pacientes poderão confirmar que o tratamento com dose baixa pertence à fronteira de eficiência, e o tratamento com dose alta é relativamente ineficiente, de acordo com o método DEA.
Resumen en ingles
Cutaneous leishmaniasis is a major public health problem in Brazil. The first choice drug for its treatment is meglumine antimoniate, which however presents considerable systemic toxicity. Treatment with high dose (20mg of pentavalent antimony - Sb5+ - per kilogram of body weight per day for 20 days) and low dose (5 mg Sb5+/kg/day for 30 days) were previously compared, without allowing conclusive results. A clinical trial of equivalence between treatment with high dose and low dose is currently underway at the Laboratory of Leishmaniasis Surveillance at Evandro Chagas Clinical Research Institute, Oswaldo Cruz Foundation, Rio de Janeiro, Brazil. We conducted a cost-effectiveness evaluation nested in this clinical trial with the first 59 patients with cutaneous leishmaniasis, using the model of data envelopment analysis (DEA) We aimed to determine, by calculating the mean scores synthesis of the applied resources (inputs) and the results (outputs) obtained from patients subjected to the different treatments, what intervention of the trial belongs to the efficient frontier, and whether any intervention is inefficient. There was no difference between the interventions of the trial, according to the comparison of the mean scores synthesis by DEA model. However, by comparing the pairs for each patient with their reference (benchmarks) on the efficient frontier, we observed that there were more patients in the treatment group with low dose than patients in the high dose group in the efficient frontier. Therefore, the treatment group with low dose was more efficient than the high dose group. Further studies with larger number of patients may confirm that low dose treatment belongs to the efficient frontier, and high dose treatment is relatively inefficient, according to the DEA method.
