Dermatófitos são fungos filamentosos queratinofílicos e estima-se que cerca de 20-25% da população mundial carrega uma infecção por dermatófitos. Trichophyton é um gênero com mais espécies patogênicas antropofílicas e Trichophyton rubrum é o agente etiológico mais prevalente das dermatofitoses no mundo, sendo em alguns casos a causa de tratamentos mais longos e muitas vezes mais complexos podendo causar desde lesões superficiais até crônicas. Para prevenção e controle das dermatofitoses, deve-se considerar a localização da lesão, agente causador e fonte da infecção. O tratamento desta doença é feito com antifúngicos como Terbinafina, Cetoconazol, Fluconazol, Anfotericina B e outros. No entanto, o uso indevido e indiscriminado de antifúngicos está causando um aumento na resistência a antifúngicos em dermatófitos. O objetivo deste estudo foi avaliar a variabilidade genética de isolados clínicos de Trichophyton obtidos de pacientes atendidos na Fundação de Medicina Tropical Heitor Vieira Dourado e verificar o perfil de susceptibilidade a nove antifúngicos; e através de uma revisão sistemática fornecer uma visão geral da prevalência mundial de Trichophyton rubrum e outros dermatófitos. As espécies T. tonsurans, T.
interdigitale, T. rubrum e T. verrucosum foram testadas quanto à resistência aos antifúngicos e todas apresentaram isolados resistentes a algum antifúngico, mas majoritariamente a resistência foi observada a Anfotericina B, Fluconazol, Miconazol e Nistatina. A análise filogenética dos dermatófitos foi realizada com a região ribossomal e a variabilidade genética foi conduzida por análise de RFLP desta mesma região. Os dados mostraram uma diversidade genética em T. tonsurans e T. interdigitale. A revisão sistemática permitiu concluir que a espécie T. rubrum tem uma ampla distribuição mundial, sendo a mais prevalente, seguido por
T. mentagrophytes, Microsporum canis, Epidermophyton floccosum e Trichophyton interdigitale.
Abstract
Dermatophytes are keratinophilic filamentous fungi and it is estimated that about 20-25% of the world population carries a dermatophyte infection. Trichophyton is a genus with more anthropophilic pathogenic species and Trichophyton rubrum is the most prevalent etiological agent of dermatophytosis in the world, being in some cases the cause of longer and often more complex treatments that can cause from superficial to chronic lesions. For the prevention and control of dermatophytosis, the location of the lesion, causative agent and source of infection should be considered. Treatment of this disease is done with antifungals such as Terbinafine, Ketoconazole, Fluconazole, Amphotericin B and others. However, excessive and indiscriminate use of antifungals is causing an increase resistance to antifungals on dermatophytes. The objective of this study was to evaluate the genetic variability of clinical isolates of Trichophyton obtained from patients attended at the Heitor Vieira Dourado Fundação de Medicina Tropical and to verify the susceptibility profile to nine antifungals; and through a systematic review provide an overview of the worldwide prevalence of
Trichophyton rubrum and other dermatophytes. The species T. tonsurans, T. interdigitale, T. rubrum and T. verrucosum were tested for antifungal resistance and all had antifungal resistant isolates, but resistance was observed for Amphotericin B, Fluconazole, Miconazole and Nystatin. The phylogenetic analysis of the dermatophytes was performed with the ribosomal region and the genetic variability was conducted by RFLP analysis of the same region. The data showed a genetic diversity in T. tonsurans and T. interdigitale. The
systematic review allowed us to conclude that the species T. rubrum has a wide world distribution, being the most prevalent, followed by T. mentagrophytes, Microsporum canis, Epidermophyton floccosum and Trichophyton interdigitale.
