As coleções hídricas são importantes cenários na disseminação e seleção de bactérias resistentes. O descarte inapropriado de dejetos e a contaminação fecal possuem fortes impactos ambientais e na comunidade, sendo esta última mais propensa a contrair infecções através destes recursos hídricos contaminados. OBJETIVO: O objetivo do trabalho foi avaliar o perfil microbiológico e de resistência antimicrobiana de coleções hídricas de área rural e urbana da Bahia. MATERIAL E MÉTODOS: Para esta análise,amostras de coleções hídricas sem proximidades a hospitais ou centros de saúde do distrito de Jenipapo em Ubaíra/BA (zona rural, n=3) e de Salvador/BA, nas comunidades de Dique do Cabrito, Dique do Tororó e Lagoa do Abaeté (zona urbana, n=9) foram avaliadas e coletadas a cada três meses no período de um ano (Out/2016-Ago/2017). A quantificação de coliformes totais foi realizada através do kit Coliscan EasyGel®. A triagem de enterobactérias foi realizada em duas placas de Ágar MacConkey, contendo 2μg/mL de cefotaxima e 1μg/mL de meropenem, individualmente. A identificação foi realizada por espectrometria de massa (MALDI-TOF®) e o perfil de sensibilidade por Vitek®. A presença dos genes de resistência aos betalactâmicos foi analisada por PCR convencional, para os isolados de Escherichia coli, Enterobacter cloacae e Klebsiella pneumoniae.Os primers tinham como alvo os genes de resistência associados à produção de betalactamases do tipo cefotaximases (blaCTX-M, blaSHV, blaTEM) e carbapenemases (blaKPC,blaVIM, blaNDM, blaSPM, blaOXA-48). RESULTADOS: A contagem média de coliformes totais para Jenipapo foi de 1409 UFC/100 mL. Nos sítios urbanos Dique do Cabrito, Dique do Tororó e Lagoa do Abaeté foram 2885, 601 e 617 UFC/100 mL, respectivamente. Foram identificadas 19 espécies de enterobactérias do total de 196 bactérias selecionadas. As principais enterobactérias identificadas na área rural foram: E. cloacae (31%), Providencia rettgerii (18%), E. coli (9%) e Morganella morgani (9%). Na área urbana: E. cloacae (38%),K. pneumoniae (27%) e E coli (16%). Mais que 35% dos isolados de E. cloacae mostraramse resistentes a ampicilina/sulbactam. Na área urbana foram ainda identificadas cepas de E. coli e K. pneumoniae multirresistentes, 20% e 7% respectivamente. Todos os isolados de P. rettgeri 12/12 (100%) e 4/4 (100%) M. morganii também demonstraram ser resistentes a ampicilina/sulbactam. Genes associados à resistência foram identificados tanto nas enterobactérias analisadas na área rural: blaVIM (38%), blaOXA-48 (33%), blaCTXM (19%) e blaSPN (9%); quanto na área urbana: blaOXA-48 (35%), blaCTX-M (22%); blaVIM (19%);blaSPN (19%) e blaNDM (2,7%); blaTEM (1,3%). CONCLUSÕES: Os resultados apresentados neste trabalho evidenciaram a presença de contaminação fecal e de enterobactérias resistentes nas coleções hídricas na Bahia, tanto do ambiente rural quanto urbano, mesmo distante de áreas hospitalares.
Abstract
INTRODUCTION: Water collections are important scenarios in the dissemination and
selection of resistant bacteria. Inadequate waste disposal and fecal contamination have
strong environmental and community impacts, the latter being more likely to contract
infections through these contaminated water resources. OBJECTIVE: The objective of this
work was to evaluate the microbiological and antimicrobial resistance profile of water
collections in rural and urban areas of Bahia. MATERIAL AND METHODS: For this analysis,
samples of water collections without proximity to hospitals or health centers of the district of
Jenipapo in Ubaíra/BA (rural area, n=3) and of Salvador/BA, in the communities of Dam of
Cabrito, Dam of Tororó and Abaeté Lagoon (urban area, n=9) were evaluated and collected
every three months in a one-year period (Oct/2016-Aug/2017). Quantification of total
coliforms was performed using the Coliscan EasyGel® kit. Enterobacteria screening was
performed on two MacConkey agar plates containing 2 μg/mL cefotaxime and 1 μg/mL
meropenem, individually. Identification was performed by mass spectrometry (MALDITOF
®) and the sensitivity profile by Vitek®. The presence of beta-lactam resistance genes
was analyzed by conventional PCR for Escherichia coli, Enterobacter cloacae and Klebsiella
pneumoniae isolates. The primers targeted resistance genes associated with the production
of beta-lactamases of the cefotaxime type (blaCTX-M, blaSHV, blaTEM) and carbapenemases
(blaKPC, blaVIM, blaNDM, blaSPM, blaOXA-48). RESULTS: The mean total coliform count for
Jenipapo was 1409 CFU / 100 mL. In the urban sites Dam of Cabrito, Dam of Tororó and
Abaeté Lagoon were 2885, 601 and 617 CFU / 100 mL, respectively. A total of 19 species
of enterobacteria were identified from a total of 196 selected bacteria. The main
enterobacteria identified in the rural area were: E. cloacae (31%), Providencia rettgerii
(18%), E. coli (9%) and Morganella morgani (9%). In the urban area: E. cloacae (38%), K.
pneumoniae (27%) and E coli (16%). More than 35% of E. cloacae isolates were resistant
to ampicillin/sulbactam. In the urban area, strains of multiresistant E. coli and K. pneumoniae
were also identified, 20% and 7%, respectively. All isolates from P. rettgeri 12/12 (100%)
and 4/4 (100%) M. morganii have also been shown to be resistant to ampicillin/sulbactam.
Genes associated with resistance were identified in both enterobacteria analyzed in the rural
area blaVIM (38%), blaOXA-48 (33%), blaCTX-M (19%) e blaSPN (9%); and in the urban area:
blaOXA-48 (35%), blaCTX-M (22%); blaVIM (19%); blaSPN (19%) e blaNDM (2,7%); blaTEM (1,3%).
CONCLUSIONS: The results presented in this study evidenced the presence of fecal
contamination and of resistant enterobacteria in the water collections in Bahia, both in rural
and urban environments, even distant from hospital area.
