CONDIÇÕES DE SAÚDE BUCAL DA POPULAÇÃO IDOSA EM UMA COMUNIDADE RURAL DO AMAZONAS

No Brasil há um elevado número de idosos em precárias condições de saúde bucal, apresentando alta prevalência de perda dentária e edentulismo, principalmente em populações rurais. Diante disto, este estudo objetivou avaliar as condições de saúde bucal do universo de 138 idosos, com 60 anos ou mais, residentes da comunidade rural Lago do Limão, Iranduba/Amazonas/Brasil. Tratou-se de um estudo transversal, observacional e descritivo realizado em 2017. A metodologia foi baseada no Projeto SB-Brasil e incluiu coleta informações sobre dados socioeconômicos, acesso aos serviços odontológicos, autopercepção em saúde bucal e condições de uso da prótese. Foram realizadas coletas de amostras biológicas do palato e das próteses mucossuportadas por meio de swabs e exames clínicos para determinar a prevalência das principais doenças bucais. A identificação das espécies de leveduras do gênero Candida foi realizada por meio de características morfológicas das colônias e meio chromogênico. Os dados foram analisados usando o software PStata e utilizou-se os testes Qui-quadrado, Exato de Fisher, McNemar, Mann Whitney e Regressão logística (p<0,05). A maioria da população era constituída do sexo masculino, de etnia parda, com ensino fundamental incompleto, casa própria, não possuindo meio de transporte próprio e com renda familiar média de R$ 853,76. O acesso aos serviços odontológicos ocorreu principalmente através do sistema de saúde privado (50,77%) e teve como principal motivo a visita de rotina. O CPO-D médio encontrado foi de 29,93 (± 3,91), com predomínio do componente
perdido (82,96%) e a prevalência de edentulismo foi de 63,08%. A condição periodontal foi
avaliada em poucos sextantes, os demais (90,64%) foram excluídos em consequência de edentulismo, e a condição mais frequente foi a presença de cálculo dental. O uso de prótese foi
mais frequente na arcada superior (83,07%), com uma maior necessidade de prótese total, tanto
para o arco superior (60,77%) quanto para o inferior (52,31%). A maioria das próteses, superiores
e inferiores, apresentaram-se inadequadas ao uso, principalmente por motivo estético e constatou-
se uma associação estatística significativa entre as próteses inadequadas e o tempo de uso das
mesmas. A avaliação micológica revelou uma maior proporção de Candida nas próteses dentárias
em relação a mucosa oral, sendo a Candida albicans a espécie mais prevalente. A avaliação da
saúde bucal autopercebida foi relatada como boa e ótima sob todos os aspectos. Esses resultados
podem subsidiar ações de saúde específicas a essa população, considerando as peculiaridades do
ambiente em que vivem.
Resumen en ingles
In Brazil, there is a high number of elderly people in precarious conditions of oral health,
presenting high prevalence of tooth loss and edentulism, especially in rural populations. Therefore, this study aimed to evaluate the oral health conditions of 138 elderly people, aged 60 years and over, living in the rural community of Lago do Limão, Iranduba / Amazonas / Brazil. This study is a cross-sectional, observational and descriptive analysis conducted in 2017. The methodology, based on SB-Brazil project, includes information on socioeconomic data, access to dental services, self-perception in oral health and prothesis use conditions. Biological samples were collected from the palate and mucosuporated prostheses through smears and were made clinical exams to determine the prevalence of major oral diseases. Yeast species of the genus Candida were identified through morphological characteristics of the colonies and chromogenic medium. Data were statistically analyzed with PS-software using Chi-square, Fisher's exact, McNemar, Mann Whitney and Logistic regression (p <0.05) methods. The majority population was male, of lighted-skinned black ethnicity, with incomplete elementary education, own house, having no means of
own transport and average personal income of R$ 853.76. Most of population access dental
services through the private health system (50.77%) and had as main reason the routine visit. The
mean DMFT found was 29.93 (± 3.91), with a predominance of the lost component (82.96%) and
a prevalence of edentulism of 63.08%. The periodontal condition was evaluated in a few sextants,
the others (90.64%) were excluded as a consequence of edentulism, and the most frequent
condition for a dental calculus presence. The use of a prosthesis was more frequent in the upper
arch (83.07%), with a greater need for total prosthesis, both for the upper arch (60.77%) and lower
arch (52.31%). Most of the prostheses, superior and inferior, were inadequate to use, mainly due
to aesthetic reasons and a statistically significant association was found between inadequate
prostheses and the time of use. A mycological evaluation revealed a higher proportion of Candida
in dental prostheses compared to oral mucosa, with a Candida albicans being the most prevalent
species. Self-perceived oral health assessment was reported as good and optimal in all respects.
These results can subsidize health actions specific to this population, considering as peculiarities
of the environment in which they live.
