INTRODUÇÃO: A cultura é considerada a metodologia padrão ouro para a identificação de N. meningitidis. Entretanto, o emprego de técnicas moleculares, como a PCR em tempo real (qPCR) tem melhorado a capacidade de detecção do meningococo. Nos estudos em portadores, a qPCR empregando o gene sodC como alvo tem sido indicada. OBJETIVO: O presente estudo teve como objetivo avaliar a técnica de qPCR (sodC) para a identificação de N. meningitidis, a partir de material de orofaringe mantido em meio de transporte STGG, coletado de portadores assintomáticos. MATERIAL E MÉTODOS: O ensaio da qPCR foi padronizado e validado com critérios locais e a metodologia comparada com os resultados obtidos previamente a partir da cultura. Foram utilizadas um total de 1200 amostras de material de orofaringe mantidas em meio de transporte STGG a -70°C. Os ensaios da qPCR foram realizados empregando o cycle threshold (Ct) padrão (Ct≤35) e o Ct≤38 validado neste estudo. RESULTADOS: Através da qPCR (Ct≤35) foram obtidas um total de 84 (7%) amostras positivas (Ct médio= 30,4 ± 3.1) em comparação com a cultura (n=59; 4,9%), enquanto a qPCR (Ct≤38) permitiu a identificação de 99 (8,3%) amostras positivas (Ct médio= 35,6 ± 5,1). Apenas uma amostra positiva na cultura não foi identificada através da qPCR (Ct≤38). A concentração de DNA na etapa prévia de extração melhorou a capacidade de detecção de amostras positivas na qPCR. A caracterização dos genogrupos de N. meningitidis através da qPCR mostrou a presença de amostras não-grupáveis. CONCLUSÔES: Os resultados obtidos neste estudo mostram que a qPCR (Ct≤38) é uma alternativa eficaz para a identificação e caracterização de N. meningitidis a partir do meio de transporte STGG, devendo ser empregada em associação à cultura nos estudos de portadores de N. meningitidis.
Abstract
INTRODUCTION: Culture is considered the gold standard methodology for the identification of N. meningitidis. However, the use of molecular techniques such as real-time PCR (qPCR) has improved the detection of meningococcus. In carrier studies, qPCR employing the target sodC gene has been indicated. AIM: The aim of our study was to assess the qPCR using sodC gene for detecting N. meningitidis in orophayngeal secretions mantained in STGG transport medium obtained from asymptomatic carriers. MATERIAL AND METHODS: The qPCR assay was standardized and validated with local criteria and the methodology was compared with the results obtained previously by classical method (culture). A total of 1200 samples of oropharyngeal material maintained in STGG transport medium at -70°C were used. The qPCR assays were performed using the standard cycle threshold (Ct) (Ct≤35) and the Ct≤38 assay validated in this study. RESULTS: A total of 84 (7%) positive samples (mean Ct = 30.4 ± 3.1) were obtained through the standard qPCR (Ct≤35) assay in comparison with the culture (n=59; 4,9%), whereas qPCR (Ct≤38) assay allowed the identification of 99 (8.3%) positive samples (mean Ct = 35.6 ± 5.1). Only a culture-positive sample was not identified through qPCR (Ct≤38). The concentration of DNA in the previous extraction step improved the ability to detect positive samples. The characterization of N. meningitidis genogroups through qPCR showed the presence of non-groupable samples. CONCLUSIONS: The results obtained in this study show that qPCR (Ct≤38) assay is an effective alternative for the identification and characterization of N. meningitidis from the STGG transport medium and it should be used in association with culture in the studies of N. meningitidis carriers.
