Para o diagnóstico sorológico da Leishmaniose Tegumentar Americana por Leishmania (Viannia) braziliensis foram comparados os desempenhos da Reação Imunoenzimática (ELISA) e da Imunofluorescência indireta (IFI), utilizando-se os antígenos de L. braziliensis e L. major-Like no ELISA e estes antígenos e também o antígeno do Kit Bio-Manguinhos ns IFI. Essas técnicas foram testadas em três grupos de pacientes: Grupo I - 94 pacientes com LTA causadas por L. (V) braziliensis; Grupo II - pacientes com IDRM negativa residentes em área endêmica; Grupo III - pacientes com outras doenças consideradas diagnósticos diferencias para LTA. As análises estatísticas no total de casos mostraram parâmetros de acurácia (sensibiblidade 95,7%; especificidade 100% VPP 100%; VPN 97,5%) e confiabilidade (ICC 0,940) para o teste ELISA com antígeno de L. braziliensis mais elevados do que com o antígeno de L. major-like (sensibilidade78,7%; espcificidade 82,8%; VPP 73,3%; VPN 86,6%; ICC 0,833). A densidade ópitca da reação ELISA, com os dois antígenos utilizados, foi maior na forma mucosa (incluída a mucocutânea) da LTA, assim como a correlação com o tempo da evolução da doença, sendo a DO mais alta com o antígeno de L. brazilensis. Para a IFI, com antígeno de L. braziliensis, a sensibilidade foi 81,5% e a especificidade 86,2%; com antígeno de L. major-like, 95,4% e 77,7%, e com o antígeno do Kit Bio-Manguinhos, 75,4% e 89,2%, respectivamente. Utilizando-se, na IFI, o antígeno de L. braziliensis, verificou-se respcetivamente, 94,1% e 76,9% de positividade entre pacientes de forma cutânea e mucosa, enquanto com antígeno de L. maior-like foi de 96,0% e 92,3% e com o antígeno Kit Bio-Manguinhos de 70,5% e 53,8%. Uso dos testes ELISA e IFI, independentemente dos antígenos, em paralelo0 elevaram a sensibilidade e os testes em série aumentaram a especificidade

We compared accuracy of serologic diagnosis of American Tegumentary Leishmaniasis (ATL) caused by Leishmania (Viannia) braziliensis by Immunoenzimatic Test (ELISA) and Indirect Imunofluorescence (IFI), using antigens of L. braziliensis, L. major-Like and Bio-Manguinhos Kit. These methods had been tested in three groups of patients: Group I - 94 patients with ATL caused by L. (v) braziliensis; Group II - patient with negative IDRM wich live in endemic area; Group III - patient with other illnesses considered differentials diagnosis for ATL. Cut-off, defined by the area under the curve-ROC, had been 0,230 to L. braziliensis antigen and 0,095 to L. major-Like antigen, using antigens in concentrations of 5,0 and 10,0 ug/mL, respectively. Statistical analyses in the total of cases had shown parameters of accuracy to ELISA test (sensitivity 95.7%; specificity 100%; PPV 100%; NPV 97.5%) and reliability of ICC 0,940, better with antigen of L. braziliensis compared of antigen of L. major-Like (sensitivity 78.7%; especificidade 82.8%; PPV 73.3%; NPV 86.6%; ICC 0,833). Optic density of ELISA Test to both antigens was bigger in the mucous form (enclosed the mucous) of ALT. We detected a positive correlation between time (in days) of evolution of the illness with DO strongest with L. braziliensis antigen than L. major-Like. Using L. braziliensis antigen, in IFI, we observed sensitivity of 81.5% and specificity of 86.2%; compared to 95.4% and 77.7% using L. major-Like antigen and 75.4% and 89.2% with Bio-Manguinhos Kit, respectively. Specificity of IFI didn\2019t differ between antigens, however sensitivity were different using L. braziliensis and L. major-Like compared to Bio-Manguinhos Kit In immunofluorescence test using L. braziliensis antigen it was verified positivity of 94.1% (cutaneous form) and 76.9% (mucous form), using L. major-Like was 96,0% and 92,3%, and was 70,5% and 53,8% using the Bio-Manguinhos Kit. Parallel use of ELISA and IFI, independently of antigens, had raised sensitivity and use in series had increased global specificity
