O conhecimento acumulado sobre o conjunto de alterações observadas no sistema imune em decorrência do envelhecimento sugere uma correlação inversa entre o status imunológico, resposta a vacinação, saúde e longevidade, com grande impacto clínico na população idosa. As principais alterações imunológicas relacionadas ao envelhecimento resultam da involução do timo e da modulação das populações leucocitárias, provavelmente como consequência de resposta imune alterada contra patógenos. Embora relevante em praticamente todas as patologias que acometem os idosos, é incontestável que a imunosenescência produz seus efeitos mais nítidos no campo das doenças infecciosas, aumentando a susceptibilidade dos indivíduos a elas, e, ainda, acentuando a morbidade e mortalidade nesse segmento populacional em comparação com as outras faixas etárias. O melhor entendimento sobre os mecanismos que aceleram as disfunções do sistema imune em indivíduos idosos pode permitir intervenções terapêuticas que previnam, ou pelo menos retardem o aparecimento de doenças relacionadas com a idade e contribuam ainda mais com o aumento da expectativa e da qualidade de vida nessa população. Assim, esse artigo tem como objetivo revisar brevemente aspectos relevantes da natureza dos marcadores da imunosenescência, e refletir sobre sua utilidade como um fator prognóstico da longevidade humana.
