Diversos estudos têm demonstrado altas taxas de letalidade e de custos gerados com as infecções por Staphylococus aureus resistente à meticilina (MRSA), sendo raros os trabalhos nacionais que identificam os fatores de risco para colonização / infecção por esta cepa, embora esses sejam de grande importância para a redução dos indicadores. Com o objetivo de estabelecer e quantificar estabelece e quantificar estes fatores, foi feito um estudo casa-controle, proveniente de uma coorte de pacientes internados de janeiro de 2002 a dezembro de 2004 no Instituto de Pesquisa Clinica Evandros Chagas \2013 IPEC / FIOCRUZ. Para o diagnóstico laboratorial foram coletados swabs nasais dos pacientes na admissão e semanalmente, além de culturas clínicas. Uma análise de regressão logística foi utilizada. Ao final foram identificadas as seguintes variáveis independentemente associadas com colonização / infecção por MRSA: internação por pneumonia: OR: 25,63 (IC: 2,21 \2013 297,76 / p = 0,010); utilização de 2 ou mais antibióticos: OR 22,14 (IC: 6,58 \2013 74,52 / p = 0,000); ser proveniente de outro hospital: OR: 12,09 (IC: 2,54 \2013 57,62 / p = 0,002); internação por aids: OR: 9,26 (IC: 1,51 \2013 5698 / p = 0,016); utilização de dispositivo vascular central: OR 7,71 (IC: 1,21 \2013 49,27 / p = 0,031); ser do sexo masculino: OR: 3,81 (IC: 1,33 \2013 10,94 / p = 0,013) e ser portador de dermatose: OR: 3,09 (IC: 1,01 \2013 9,50 / p = 0,049). Entre os antibióticos foram identificados clindamicina: OR: 8,57 (IC: 2,01 \2013 36,49 / p = 0,004); cefalosporina de 3ª ou de 4ª geração: OR: 7,71 (IC: 2,03 \2013 29,31 / p = 0,003) e rifampicina: OR: 4,30 (IC: 1,56 \2013 11,85 / P = 0,005). Nenhuma variável relacionada ao ambiente hospitalar foi demonstrada como fator de risco Consideramos que a detecção destes fatores poderá otimizar a implantação de medidas preventivas, bem como o diagnóstico e o tratamento precoces, úteis para o controle da infecção nosocomial no IPEC. Embora não se possa extrapolar estas estimativas, a determinação de fatores de riscos pode ser útil para orientar outras instituições nas quais estes estudos seriam muito difíceis de serem conduzidos, por requererem diferentes recursos
Abstract
Studies have shown high lethality rates and costs caused by methicillin-resistant Staphylococcus aureus (MRSA) infection, but the identification of risk factors for colonization/infection by this microorganism has not been done in Brazilian facilities, although they are very important for reducing the rates. With the aim of establishing and accounting these factors, a case-control study was made, based on a cohort of patients of Instituto Nacional de Infectologia Evandro Chagas \2013 IPEC / FIOCRUZ, from January 2002 to December 2004. Nasal swabs were collected on the admission and weekly during the hospitalization and also clinical cultures were performed The logistic regression was used for statistical analysis. The following variables were identified as independently associated with MRSA colonization / infection: diagnosis of pneumonia on admission: OR: 25,63 (CI: 2,21 \2013 297,76 / p = 0,010); use of 2 or more antibiotics: OR: 22,14 (CI: 6,58 \2013 74,52 / p = 0,000); transferred from another hospital: OR: 12,09 (CI: 2,54 \2013 57,62 / p = 0,002); aids as admission diagnosis: OR: 9,26 (CI: 1,51 \2013 56,98 / p = 0,016); central vascular device: OR: 7,71 (CI: 1,21 \2013 49,27 / p = 0,031); male gender: OR: 3,81 (CI: 1,33 \2013 10,94 / p = 0,013) and skin diseases: OR: 3,09 (CI: 1,01 \2013 9,50 / p = 0,049). Among the antibiotics were identified clindamycin: OR: 8,57 (CI: 2,01 \2013 36,49 / p = 0,004); 3rd or 4th generation cephalosporins: OR: 7,71 (CI: 2,03 \2013 29,31 / p = 0,003) and rifampin: OR: 4,30 (CI: 1,56 \2013 11,85 / p = 0,005). None of the variables related to hospital environment were detected as risk factors. Preventable measures and the earlier diagnose and treatment could be guided by these risk factors to control nosocomial infections on IPEC. Although these estimates cannot be surpassed, these factors can be used by other institutions, where such studies would be difficult to be conducted, because they require diverses resources
