ESTUDO COMPARATIVO DE SISTEMAS REVELADORES EM ENSAIOS IMUNOENZIMÁTICOS PARA DETECÇÃO DE ANTICORPOS ESPECÍFICOS NO DIAGNÓSTICO DA LEISHMANIOSE TEGUMENTAR AMERICANA

O presente trabalho tem como objetivo comparar a acurácia e confiabilidade de três sistemas reveladores de ensaios imunoenzimáticos na detecção de anticorpos IgG específicos para o diagnóstico da LTA causada por Leishmania (Viannia) braziliensis. Trata-se de um estudo diagnóstico, mascarado, com repetições independentes, em população total de 378 pacientes de um ambulatório de referência no Rio de Janeiro, dos quais 189 amostras de soro de pacientes com diagnóstico parasitológico positivo para LTA e 189 com diagnóstico confirmado para outras doenças cutâneas do espectro de diagnóstico diferencial de LTA. Nos ensaios imunoenzimáticos foram empregados antígenos parcialmente solúveis obtidos de formas promastigotas de L. (V.) braziliensis e na revelação, dois cromógenos, 1,2-ortofenilenodiamino (OPD) e 3,3\2019,5,5\2019-tetrametilbenzidina (TMB) e um fluorógeno, 4-metilumbeliferilfosfato (MUP). O desempenho de cada sistema revelador foi comparado através dos seus parâmetros de acurácia, Coeficiente de Correlação Intraclasse (ICC) e as áreas sob a curva ROC (Receiver Operating Characteristic). A sensibilidade foi idêntica (97,4%) para todos os sistemas. A confiabilidade foi excelente (ICC = 98,61%; 98,74% e 99,11%) e a especificidade variou de 93,7%, 95,8 a 97,4% para OPD, MUP e TMB, respectivamente, sem significância estatística. A não diferença entre as excelente performances dos três reveladores sugere que se utilize o TMB por possuir vantagens de ordem operacionais como baixo custo referente ao equipamento de leitura, fácil manipulação, estabilidade e menor toxicidade

The present work has the objective to compare accuracy and reliability of three revealing systems of immunoenzymatic assays in the detection of specific IgG antibodies for the diagnosis of ATL caused by Leishmania (Viannia) braziliensis. This is a diagnostic, masked study, with independent repetitions, in a total population of 378 patients of a reference clinic in Rio de Janeiro, of which 189 serum samples of patients with positive parasitological diagnosis for ATL and 189 with diagnostic confirmed for other cutaneous pathologies of the spectrum of differential diagnosis with ATL. The immunoenzymatic assays was performed using partially soluble antigens obtained from promastigote forms of L. (V.) braziliensis and as substrates, two chromogenics, 1,2-o-phenylenediamine (OPD) and 3,3',5,5' - tetramethylbenzidine (TMB) and one fluorogenic, 4- methylumbelliferyl phosphate (MUP) Performance of each revealing system was compared through its accuracy parameters, Intraclass Correlation Coefficient (ICC) and the areas under the ROC curve (Receiver Operating Characteristic). Sensitivity was identical (97.4%) for all systems. Reliability was excellent (ICC = 98.61%, 98.74% and 99.11%) and specificity varied of 93,7%, 95.8% and 97.4% for OPD, MUP and TMB, respectively, without statistically significance. The lack of difference between the excellent performances of these three substrates suggests the usage of TMB because its operational advantages like low cost of the reading equipment, easy manipulation, stability and less toxicity
