ESTUDO DA PREVALÊNCIA DAS ALTERAÇÕES COGNITIVAS LIGADAS AO HIV EM PACIENTES PORTADORES DA INFECÇÃO PELO HIV/AIDS

Introdução: O comprometimento cognitivo relacionado à infecção pelo HIV é descrito desde 1987. Inicialmente denominado complexo demência \2013 AIDS, o quadro apresentava caráter progressivo sendo altamente limitante. Depois do advento da HAART em 1996, a prevalência desta forma clínica sofreu queda em seus índices, porém outros graus de comprometimento cognitivo foram reconhecidos e, em 1997 a AAN estabeleceu uma nova terminologia para classificar um espectro clinico mais abrangente, incluindo as formas mais brandas. Apesar da HAART ter reduzido a morbidade e a mortalidade da infecção pelo HIV de maneira relevante, prevalências tão altas quanto 50% ainda tem sido descritas para as HAND. Não há, na literatura, dados referentes aos déficits cognitivos relacionados ao HIV para pacientes brasileiros. Para esclarecer este ponto, a partir de uma coorte nacional, avaliamos a prevalência dos distúrbios neurocognitivos relacionados ao HIV em todas as suas manifestações, ponderando sobre alguns fatores (idade, gênero, nível educacional, CD4+ nadir e carga viral) possivelmente associados à sua ocorrência. Objetivos: Avaliar a prevalência dos distúrbios neurocognitivos em pacientes portadores da infecção HIV / AIDS , considerando todas as suas formas clínicas e os fatores a elas relacionados, além de avaliar a escala de demência internacional como possível ferramenta de rastreio para diagnóstico precoce das alterações neurocognitivas relacionadas ao HIV Material e métodos: Para um estudo propectivo, foram selecionados aleatoriamente 187 pacientes portadores do vírus HIV / AIDS (53,48% homens / 38,50% VT ), provenientes dos ambulatórios da F ioc ruz e do Hospital M unicipal Dese mbargador Leal Junior , se guindo critérios de exclusão pré-determinados. Parte dos pacientes (89/187) foi submetida à escala de demência internacional (EDI HIV ) por um neurologista e um fisioterapeuta, em momentos distintos, num intervalo máximo de 14 dias. A seguir, foram avaliado s cinco domínios cognitivos através da aplicação de uma bateria de testes neuropsicológicos pelo neurologista. Comparamos os resultados da EDI HIV com os obtidos através dos TNP , e posteriormente, realizamos uma avaliação interprofissional através da compar ação dos resultados obtidos pela aplicação da EDI HIV pelo neurologista e pelo fisioterapeuta. Todos os dados foram analisados através do SPSS 17. Resultados: O déficit cognitivo foi verificado em 5 2, 4 0 % dos pacientes avaliados. Seguindo os critérios da AAN, 71,5% apresentavam co mprometimento cognitivo leve e 28,5 % já apresentavam quadro demencial. Dentre os fatores de associaç ão avaliados, a escolaridade (p - valo r = 0,047 )) e o gênero (p valor= 0,030 ), foram os considerados estatisticamente relevantes. A EDI HIV teve grau de con cordância interprofissional de 0,684 e , quando comparada aos TNP , mostrou especificidade de 77% e sensibilidade de 57% Conclusões: A prevalência das alterações cognit i vas relacionadas ao HIV / AIDS na população estudada é compatível com os dados disponíveis na literatura referentes a estudos internacionais. A EDI HIV é de fácil manuseio, demora apenas de 2-3 min,tem a vantagem de poder ser aplicada por não especialistas, em ambiente ambulatorial, nã o requer material específico e é capaz de detectar dano sub cortical como ocorre na demência pelo HIV . No entanto, não pode ser considerada como método substituto à bateria de TNP no diagnóstico de demência pelo HIV , apresentando limitações para avaliação de casos mais sutis e/ou incipientes de comprometimento cognitivo
Abstract
Introduction: Cognitive disorders related to HIV infection have been described since 1987. Initially named HIV dementia complex, its clinical syndrome was characterized as a progressive and very restrictive dysfunction. After HAART introduction in 1996, the prevalence of HIV associated dementia clearly decreased, but other degrees of cognitive impairment were recognized and in 1997, AAN developed a new terminology to classify a broadening clinical spectrum of neurocognitive impairment, including milder abnormalities. Although HAART has greatly reduced medical morbidity and mortality with HIV infection, rates as high as 50% of HIV-associated neurocognitive disorders (HAND) continue to be reported. No data is available regarding cognitive impairment in HIV Brazilian patients. In order to clarify this issue, from a national cohort, we evaluated all kinds of neurocognitive impairment presentation and its prevalence, roaming over some factors (age, gender, educational level, CD4+ nadir and viral load) that could possibly be related to its occurrence. Objectives: To evaluate the neurocognitive disorders presented by a national cohort of HIV/AIDS patients, determining its prevalence, clinical correlates and possible risk factors, and also to evaluate the International Dementia Scale as a possible screening tool to earlier identify individuals at risk for HIV dementia Methods and Material: For a prospective study 187 HIV/ AIDS outpatients from the Clinic of Infectious Diseases at Fiocruz and at the Hospital Municipal Desembargador Leal Junior (53,50% male / 38,50% without HAART use) were randomized based on established selection criteria. Part of the subjects (89/187) underwent evaluation by means of IHDS (International HIV Dementia Scale), administered by a neurologist and by a physiotherapist with a maximum gap of 14 days. Later, all participants completed a screening for comprehensive neuropsychological (NP) battery administered by a neurologist, covering five cognitive domains. Results for each of the neuropsychological tests were compared with the IHDS´s scores. To achieve an interprofessional appraisal, we compared the IHDS´s scores obtained by the neurologist and by the physiotherapist. Data were analyzed using SPSS 17. Results: Cognitive impairment was verified in 52,40% of the selected patients. According to the AAN terminology, 71,50% of them presented a Mild Neurocognitive Disorder while the other 28,50 % already met criteria for HIV associated Dementia. Among the risk factors evaluated, educational level (p-valor = 0,047) and gender (p- valor= 0,030) were considered statistically relevant. IHDS´s interprofessional agreement level was 0,684 and when compared with the NP, its sensitivity was 57%, while its specificity was 77%. Conclusion: The prevalence of HAND in our population is similar to data available regarding international cohorts. IHDS it is easy to perform, requires only 2\20133 minutes, can be provided by nonneurologists, in an outpatient setting, requires no special instrumentation, and detects subcortical damage, such as HIV dementia. However, it should not be used as a replacement for a full neuropsychological testing to confirm a diagnosis of HIV dementia. To diagnose milder forms of cognitive impairment, it has limitations
