Esta dissertação realiza uma avaliação dos Centros de Atenção Psicossocial, tipologia II para
adultos (CAPS II) do Estado do Rio de Janeiro. Para este fim, utilizou um formulário que
contempla a coleta de informação sobre cobertura, a disponibilidade de recursos humanos, a
estrutura física, a capacidade instalada, os recursos materiais e os processos de trabalho das
equipes. A revisão das pesquisas de cunho qualitativo e participativo foi essencial para a eleição
dos indicadores e categorias que compuseram o instrumento de avaliação. A opção pela
estratégia do formulário foi alcançar um número maior de unidades e obter uma visão de
conjunto dos CAPS II do Rio de Janeiro a partir da informação dos diretores das unidades. Uma
das principais evidências do estudo é a grande disparidade no padrão de implantação dos CAPS
II na cidade do Rio de Janeiro e nos demais Municípios. A adequação da cobertura e a
capacidade de acolher a demanda potencial aos novos serviços são bastante desiguais nos dois
grupos de Municípios. O efeito da escala populacional dos Municípios afeta a capacidade de
oferta dos novos serviços de saúde mental. A pesquisa mostra grande disparidade na
disponibilidade de recursos humanos e na inclusão de novos usuários aos serviços, resultando em
expressivo número destes em pronto-atendimento, em unidades especialmente da Capital. A
supervisão clínico-institucional é um dispositivo presente nos CAPS da Capital, mas pouco
implantado nos CAPS dos demais Municípios. As equipes trabalham de modo interdisciplinar,
discutindo casos clínicos com frequência e realizando pelo menos um seminário de planejamento
interno por ano. Foram identificadas barreiras de acesso aos medicamentos de segunda geração.
A pesquisa identifica um padrão organizacional em relação aos pacientes de longa permanência
(LP): 90% dos CAPS II informaram possuir estratégias para a inserção desses usuários. Chama a
atenção o significativo trabalho de desinstitucionalização realizado pelos serviços. Existem
unidades que acompanham um número expressivo de pacientes enquanto outras ainda
desenvolvem um trabalho residual com essa clientela. A proporção média de usuários de LP em
relação aos matriculados é de 4%. A inserção dos serviços na comunidade é uma das limitações
dos novos serviços. É informada a baixa participação dos usuários e familiares nos projetos
institucionais. Uma proporção expressiva de CAPS II utiliza os leitos dos hospitais psiquiátricos
conveniados para as internações e é extremamente baixo (3%) o apoio dos leitos dos CAPS III,
indicando a falta destes novos serviços no Estado do Rio de Janeiro.
Abstract
This dissertation performs an assessment of Centros de Atenção Psicossocial, type II for adults
(CAPS II) in the State of Rio de Janeiro. To this end, it used a form that includes the collection
of information on coverage, the availability of human resources, physical infrastructure, the
installed capacity, the resources and work processes of teams. A review of qualitative and
participatory research was essential for the selection of indicators and categories that made up
the assessment tool. The choice of the strategy in the assessment instrument was to achieve a
greater number of units and get an overview of CAPS II, in Rio de Janeiro, from the information
of the directors of the units. One of the main findings of the study is the large disparity in the
pattern of deployment of CAPS II in the city of Rio de Janeiro and other cities of the State. The
adjustment of coverage and capacity to accommodate the potential demand for new services are
markedly unequal in the two groups of municipalities. The effect of population size affects the
ability of municipalities to offer the new mental health services. The research shows wide
disparity in the availability of human resources and adding new services to users, resulting in
significant number of users in the emergency room, especially in units of capital. The clinicalinstitutional supervision is a device present in CAPS in the capital, but little implemented in
CAPS of the other municipalities. Teams work in an interdisciplinary way, often discussing
clinical cases and performing at least one internal planning workshop by year. Barriers to access
to second generation medicines were identified. The research identifies an organizational pattern
in relation to long-stay patients (LP): 90% of CAPS II reported having strategies for inserting
these users in community-based non-hospital treatment. The research draws attention to the
important work of deinstitutionalization of services. There are units that come with very
impressive numbers while others still develop a residual working with this clientele. The average
proportion of users enrolled in relation to the LP is 4%. The integration of services in the
community is one of the limitations of the new services. It noted the low participation of users
and families in institutional projects. A significant proportion of CAPS II use the beds in mental
hospitals covered for internment and the support for beds in CAPS III is extremely low (3%),
indicating a lack of these new services in the State of Rio de Janeiro.
