Introdução: O tétano acidental possui distribuição mundial e alta letalidade. O objetivo deste estudo foi caracterizar os casos de tétano confirmados em Pernambuco e identificar fatores associados ao óbito destes pacientes. Métodos: Estudo transversal, analítico, de dados secundários, com grupos internos de comparação. Para análise dos dados foi utilizado o teste Qui-Quadrado de Pearson e estimada a Odds Ratio com seus respectivos intervalos de confiança ao nível de 95 por cento. Na análise multivariada foi aplicada a regressão logística múltipla. Resultados: A incidência de tétano variou entre 0,65 e 2,79 casos por 1.000.000 habitantes. Maior acometimento de adultos, masculino e residente na zona urbana. Apenas 13,8 por cento tinham recebido a vacina e 23,4 por cento a receberam como profilaxia. A média da letalidade foi de 20,4 por cento. Na análise multivariada, foi associado a maior letalidade: ser do sexo feminino (p=0,051); não ter feito nenhuma profilaxia (p=0,096); e ter rigidez de nuca como sintoma (p=0,029). Conclusão: Existe a necessidade de maiores investimentos para a prevenção da doença, tanto na cobertura vacinal quanto na aplicação da profilaxia. Apesar da garantia de hospitalização com cuidados adequados, houve aumento da letalidade do tétano, o que sugere que estudos mais específicos devem ser desenvolvidos para investigar o que pode estar influenciando esta mudança. Atenção especial deve ser oferecida aos casos do sexo feminino e aos que apresentem rigidez de nuca como sintoma, pois estes grupos apresentam uma maior chance de evoluir para o óbito
Abstract
Introduction: Tetanus has a worldwide distribution and high lethality. The goal of this study was to characterize the tetanus cases confirmed in Pernambuco and identify factors associated with death in these patients. Methods: Cross-sectional, analytical study of secondary data withinternal comparison groups. For data analysis we used the Pearson’s chi-square test to estimate the odds ratios and their respective confidence intervals at 95%. Multivariate analysis was applied to multiple logistic regression. Results: The incidence of tetanus ranged between 0.65 and 2.79 cases per 1.000.000 inhabitants. With higher involvement of adults, male and city population. Only 13.8% had received the vaccine and 23.4% received it as prophylaxis. The average mortality rate was 20.4%. In multivariate analysis, female gender (p = 0.051), had made no prophylaxis (p = 0.096) and presented neck stiffness as a symptom (p = 0.029), which was associated with increased letality. Conclusion: There is a need for greater investment to prevent of the disease, both in immunization coverage and prophylaxis methods. Despite the hospitalization with proper care guaranteed, there was an increase in tetanus mortality, suggesting that more specific studies should be conducted to investigate what may have been influencing this change. Special attention should be offered to female cases that presented neck stiffness as a symptom, because these groups have a higher chance of progressing to death.
