CARACTERIZAÇÃO DE SUBPOPULAÇÕES DE LINFÓCITOS B E FUNCIONALIDADE DE ANTICORPOS ESPECÍFICOS PARA LEISHMANIA EM PACIENTES CURADOS DE LEISHMANIOSE CUTÂNEA

Estudos anteriores do nosso grupo demonstraram que os títulos séricos de IgG1 e IgG3 anti-Leishmania tendem a decair após a cura clínica da leishmaniose cutânea (LC). Entretanto, há indivíduos que mantêm títulos elevados, mesmo longo tempo após a cura. Nossa hipótese é que o perfil de subpopulações de linfócitos B que permanece após cura clínica difere entre grupos com sorologia positiva e negativa, e que essas células podem secretar anticorpos e produzir citocinas que modulam negativamente a resposta imune celular, podendo favorecer a persistência parasitária. Desta forma, o objetivo deste trabalho foi investigar se linfócitos B e anticorpos secretados desempenham um papel modulador da resposta imune celular na infecção por Leishmania (Viannia) braziliensis, em indivíduos curados longo tempo após o tratamento. Os voluntários foram agrupados de acordo com a positividade plasmática de IgG, IgG1 e IgG3 anti-Leishmania como positivos (Ig+, n=17) e negativos (Ig-, n=16), além de um grupo controle (n=19). As células mononucleares de sangue periférico (PBMC), previamente criopreservadas, foram descongeladas e colocadas em cultura por cinco dias na ausência de estímulo ou na presença de antígenos de L. braziliensis (Lb-Ag) ou Toxoplasma gondii. A concentração de IgG1 ou IgG3 no sobrenadante de cultura foi quantificada por ELISA. Independente do resultado sorológico, 20% (6/30) dos indivíduos apresentaram níveis de IgG1 acima do valor de corte, para as culturas estimuladas com Lb-Ag e para as não estimuladas, enquanto que , para IgG3, foram 90% (27/30) para as culturas não estimuladas e 53% (16/30) para as estimuladas com Lb-Ag Entretanto, essas concentrações de anticorpos não apresentaram correlação com os títulos das amostras de soro dos mesmos indivíduos. O perfil de subpopulações de linfócitos B (naive, memória inicial, memória e plasmócitos) foi investigado por citometria de fluxo e não foi significativamente diferente entre os grupos Ig+ e Ig-, e entre os indivíduos curados e o grupo controle. A influência de anticorpos opsonizantes na atividade anti-Leishmania de macrófagos foi avaliada através da infecção de macrófagos por promastigotas de L. braziliensis que foram incubadas com soro de indivíduos Ig+ e Ig-. Não foi observada diferença estatística entre os grupos Ig+ e Ig- em relação à taxa e índice de infecção, produção de citocinas inflamatórias e produção de nitrito. Em suma, os indivíduos curados de LC Ig+ e Ig- são semelhantes entre si, no que diz respeito às subpopulações de linfócitos B de sangue periférico, e em relação à influência de anticorpos opsonizantes na atividade anti-Leishmania de macrófagos

Previous studies of our group demonstrated that serum anti-Leishmania IgG1 and IgG3 titles tend to decay after cutaneous leishmaniasis clinical cure (CCL). Besides this fact some CCL subjects keep high titers of specific antibodies, even after long time of cure. Our hypothesis says that circulating B lymphocytes profile, which remain after clinical cure, are different among groups with positive and negative serology and that these cells can secret antibodies that can lead to a negative modulation of cell immune response and also may favor parasitic persistence. The objective of this study was to investigate whether B lymphocytes and secreted antibodies play a role in modulating the cellular immune response in Leishmania (Viannia) braziliensis infection on subjects who were declared cured after a long time of treatment. Volunteers were distributed into CCL positive group (Ig+, n=17), CCL negative group (Ig-, n=16) according to IgG, IgG1 and IgG3 anti-Leishmania plasmatic titre, and a control group (Control, n=19). Peripheral blood monocluclear cells (PBMC) previously frozen were then thawed and kept in culture during five days in the absence of stimulus or in the presence of L. braziliensis (Lb-Ag) or Toxoplasma gondii antigens. The concentration of IgG1 and IgG3 on culture supernatants was quantified by ELISA. Regardless of serologic results, 20% (6/30) of individuals showed IgG1 levels above cut off, for stimulated cultures with Lb-Ag and for not stimulated cultures, whereas for IgG3 it was 90% (27/30) for not stimulated cultures and 53% (16/30) for stimulated cultures with Lb-Ag However, this antibodies concentration showed no correlation with the titers observed in the serum samples from the same individuals. B lymphocytes subsets profiles (naïve, early memory, memory and plasma cells) was investigated through flow citometry and was not significantly different among Ig+ and Ig- groups, and among CCL and control group. The opsonized antibodies influence in the anti-Leishmania macrophage activity was evaluated through the infection of macrophages with L. braziliensis promastigotes that were incubated with serum of Ig+ and Ig- CCL individuals. It was not observed difference among Ig+ and Ig- groups with respect to infection tax, infection index, inflammatory cytokine production, and nitrite production. This suggests that opsonizing antibodies seems not to influence anti-Leishmania macrophage activity. In summary, the CCL Ig + and CCL Ig- individuals are similar about B lymphocyte phenotype, and similar too with respect to the influence of opsonizing antibodies on the macrophages\2019 anti-Leishmania activity
