A leishmaniose tegumentar americana (LTA) é uma parasitose, de grande importância para saúde pública e que acomete pele e mucosas, causada por espécies distintas de protozoários do gênero Leishmania. A técnica de reação em cadeia da polimerase (PCR), com sua elevada sensibilidade, constitui uma ferramenta valiosa para estudos clínicos e epidemiológicos em leishmanioses. Em ensaios de PCR é crucial que o DNA alvo esteja bem purificado, a fim de que o resultado seja fidedigno. A comparação entre protocolos para extração e purificação de DNA assegura uma análise mais aprofundada da eficiência de cada um, e pode levar ao desenvolvimento de técnicas mais específicas e sensíveis que complementadas com abordagens não invasivas de coleta constituiriam um relevante avanço para o diagnóstico molecular e para a pesquisa da leishmaniose. Neste estudo comparativo, inicialmente utilizamos como substratos à extração e purificação de DNA o material genético de promastigotas resultantes de cultura de Leishmania (V.) braziliensis e de swabs embebidos em diferentes diluições da referida cultura. Durante o estudo, avaliamos os protocolos testados (kit comercial, precipitação por cloreto de sódio, extração e purificação por fenol/clorofórmio e adsorção à sílica em suspensão) sob diferentes parâmetros. Entre eles podemos citar: rendimento da extração, grau de pureza do DNA extraído, reprodutibilidade, praticidade e relação custo/benefício do processo. Mesmo sendo um dos protocolos mais conhecidos, o protocolo de extração e purificação por fenol/clorofómio só obteve vantagem no parâmetro de grau de pureza do DNA extraído. Nos outros parâmetros analisados o protocolo que mais se destacou foi o de adsorção à sílica em suspensão, obtendo resultados estatisticamente significativos (p<0,05), gerando também os melhores resultados visuais no que diz respeito aos produtos de PCR. Recomendamos, portanto, que o protocolo de adsorção à sílica em suspensão venha a ser utilizado para o diagnóstico e pesquisa da LTA, em conjunção com o método de coleta por swab, usado como um método alternativo e/ou complementar às diversas técnicas já estabelecidas de coleta de amostras dos pacientes com suspeita da doença.
Abstract
American cutaneous leishmaniasis (LTA) is a parasitic disease of great importance to public health. Affecting the skin and mucous membranes, it’s caused by different species of Leishmania protozoa. The polymerase chain reaction (PCR), with its high sensitivity, it’s a valuable tool for clinical and epidemiological studies on leishmaniasis. In PCR assays it’s crucial that the target DNA is properly purified, so that the result is reliable. At the same time, the comparison between protocols for DNA extraction and purification ensures a more in-depth analysis of their efficiency and may lead to the development of more specific and sensitive techniques that complemented with non-invasive collection approaches would constitute a relevant advance for molecular diagnosis and research of leishmaniasis. In this comparative study, initially we used as substrates for DNA extraction and purification material from promastigotes resulting from culture of Leishmania (V.) braziliensis and swabs soaked in different dilutions of that culture. During the study, we evaluated the protocols tested (commercial kit, sodium chloride precipitation, extraction and purification by phenol/chloroform and adsorption to suspended silica) under different parameters. These include: performance of the extraction and purity of the extracted DNA, reproducibility, practicality and cost / benefit rate of the process. Even one of the most known protocols, the extraction and purification phenol/chloroform protocol only got advantage in the purity grade parameter. In the other analyzed parameters the most outstanding protocol was that of adsorption to suspended silica, obtaining statistically significant results (p <0.05), which also generated the best visual results with respect to PCR products. We recommend, therefore, that the silica adsorption protocol be used for the diagnosis and research of the LTA, in conjunction with the swab collection method, used as an alternative and/or complementary method to the several established techniques of collection of samples of the patients with suspected disease.
