A maioria dos fármacos que compõe o esquema básico para o tratamento da tuberculose (TB), apesar de altamente eficazes contra o bacilo da TB, frequentemente causam hepatotoxicidade. As enzimas Glutationas S- transferases (GSTs) atuam no metabolismo de fármacos e fazem parte dos processos de detoxificação de metabólitos. Muitos estudos já investigaram as variantes nulas de GST Mu1 (GSTM1) e de GST Theta1 (GSTT1) como possíveis fatores de risco da hepatotoxicidade induzida por medicamentos anti-tuberculose. Porém ainda são poucos os estudos conduzidos em populações brasileiras, e a maioria das pesquisas com este tema não investigam polimorfismos genéticos juntamente com fatores ambientais no desenvolvimento de hepatotoxicidade. Diante disso, este estudo analisou os genótipos GSTT1 e GSTM1 nulos em pacientes brasileiros em tratamento para tuberculose controlando outros possíveis preditores que possam ter efeito na hepatotoxicidade induzida por medicamentos. Este foi um estudo prospectivo com 177 pacientes que receberam tratamento para TB no período de março de 2006 a 2011, na Fundação Oswaldo Cruz (Fiocruz), Instituto de Pesquisa Clínica Evandro Chagas (IPEC), Rio de Janeiro, Brasil. Após a obtenção do DNA genômico, as deleções de GSTM1 e GSTT1 foram analisadas através da técnica da Reação em Cadeia da Polimerase (PCR). Os dados demográficos, clínicos e laboratoriais dos pacientes foram obtidos a partir dos prontuários clínicos e armazenados em banco de dados. A mediana de idade (intervalo interquartílico) da população foi de 37 (30,0 \2013 49,0) anos, a maioria era do sexo masculino com etnia não-branca e com menos de dois parceiros sexuais por ano. As formas clínicas da TB mais freqüentes foram a pulmonar, seguida da extrapulmonar e disseminada. A co-infecção com Vírus da Imunodeficiência Humana (HIV) estava presente em 44,63% dos pacientes e 79,75% estavam sob tratamento anti-retroviral. Do total de pacientes, a prevalência de infecção ativa pelo Vírus da Hepatite B (HBV) foi de 2,26% e da infecção pelo Vírus da Hepatite C (HCV) foi de 6,78%. Ainda, 29,38% eram fumantes e 15,8% foram classificados como alcoólatras. As freqüências encontradas dos genótipos nulos foram 31,07% para GSTM1 e 22,03 % para GSTT1. A incidência de hepatotoxicidade foi 33,33% e possíveis preditores dessa reação adversa foram avaliados. A infecção ativa com HBV e os níveis elevados da alanina aminotransferase (ALT) pré-tratamento representaram preditores. As variantes genéticas analisadas não foram preditoras da hepatotoxicidade, porém há evidência de que os polimorfismos genéticos de GSTM1 estejam relacionados à intensidade da toxicidade.
