As leishmanioses são doenças que acometem milhões de pessoas ao redor do mundo, sendo considerado um grave problema de saúde pública. Devido à ausência de uma vacina eficaz e debilitante quimioterapia, melhores terapias são urgentemente necessárias. Assim, tem-se estimulado a busca por tratamentos alternativos, como por exemplo, o uso de medicamentos fitoterápicos. Diversos estudos realizados com Morinda citrifolia Linn. mostraram várias atividades biológicas desta planta, tais como: antitumoral, imunomodulatória e leishmanicida, contudo os seus mecanismos de ação permanecem desconhecidos. O objetivo do presente estudo foi avaliar in vitro e in vivo a atividade leishmanicida e os efeitos imunomoduladores do suco do fruto de M. citrifolia (noni). O suco foi obtido após deposição do fruto por 3 dias em garrafa estéril, o líquido obtido foi centrifugado e o sobrenadante liofilizado e estocado a -20 °C até o uso. Os experimentos in vitro de citotoxicidade foram realizados com macrófagos peritoneais de camundongos e os de atividade leishmanicida e avaliação ultraestrural com macrófagos J774.G8. O suco de noni apresentou um rendimento de 6,31%. A cromatografia líquida de alta eficiência identificou compostos aromáticos e fenólicos como os principais componentes e a análise de espectrometria de massa identificaram a presença de ácido desacetilaspelurosídico, ácido aspelurosídico, rutina, noniosídeo B e noniosídeo C no suco. Os valores de IC50 foram 275.3\03BCg/mL para promastigotas e 208.4\03BCg/mL para amastigota intracelular de L amazonensis, e 260.5\03BCg/mL para promastigotas e 201.3\03BCg/mL para amastigota intracelular de Leishmania infantum. Ensaio de citotoxicidade com macrófagos J774.G8 mostraram que o suco de noni não apresenta toxicidade até 2mg/mL
Formas promastigotas tratadas com suco de noni e analisadas em microscopia eletrônica de transmissão apresentaram alterações ultraestruturais como vacuolização citoplasmática, inclusão lipídica, aumento de atividade exocítica em L. amazonensis e L. infantum, além de vesículas semelhantes à autofagossomo em L infantum. O tratamento in vitro com noni levou a um aumento da produção de óxico nítrico em macrófagos peritoneais infectados e não infectados por L. amazonensis. Quando os macrófagos foram pré-tratados com aminoguanidina, um inibidor seletivo de iNOS, o número de amastigotas intracelulares de L. amazonensis aumentou, demonstrando o papel do noni na modulação da produção de NO. O tratamento in vivo com 500 mg/kg de noni administrado diariamente por gavagem durante 60 dias diminuiu o tamanho da lesão e a carga parasitária na pata e no linfonodo drenante de camundongos C57BL/6 infectados com L. amazonensis. No local da infecção ocorreu diminuição de infiltrado inflamatório e menor expressão de IL-12, TNF-\03B1, TGF-\03B2 e IL-10, quando comparado com os camundongos infectados e não-tratados. Por outro lado, o tratamento com noni aumentou a expressão de colágeno tipo IV, fibronectina e laminina na pata infectada bem como colágeno do tipo I e tipo III, fibronectina e laminina na pata de camundongos normais. Nenhuma toxicidade foi observada com o uso deste tratamento. Estes dados demonstram a eficácia do tratamento de noni, assim como seu efeito imunomodulador, e um futuro potencial para o tratamento da leishmaniose.
Abstract
Leishmaniasis is a complex disease that is considered a serious public health problem. Due to the absence of an effective vaccine and debilitating chemotherapy a better therapies are urgently needed. Thus, it has been stimulate the search for alternative treatments such as the use of herbal medicines. Several studies conducted with Morinda citrifolia Linn. have shown various biological activities such as antitumor, immunomodulation and antileishmanial activity, however its mechanisms of action are still unknown. This study aimed to analyze in vitro and in vivo the leishmanicidal activity and the immunomodulatory effects of M. citrifolia fruit juice (noni). The juice was obtained after deposition of the fruit for 3 days in sterile bottle, the liquid obtained was centrifuged and the supernatant lyophilized and stored at -20 °C until use. The in vitro cytotoxicity experiments were carried out with mice peritoneal macrophages while the leishmanicidal activity and ultraestrural evaluation were carried out with J774.G8 macrophages. M. citrifolia fruit extract showed a yield of 6.31%. High performance liquid chromatography identified phenolic and aromatic compounds as the major constituents, and mass spectrometry analysis of the juice identified the presence of deacetylaspelurosidic acid, aspelurosidic acid, rutin, noniosideo B and noniosideo C. The IC50 values were 275.3\03BCg/mL for promastigotes and 208.4\03BCg/mL for intracellular amastigotes of L amazonensis, and 260.5\03BCg/mL for promastigotes and 201.3\03BCg/mL for intracellular amastigotes of Leishmania infantum Cytotoxicity assay with J774.G8 macrophages showed that M. citrifolia fruit juice was not toxic up to 2000\03BCg/mL. Promastigotes forms treated with M. citrifolia fruit juice showed in transmission electron microscopy alterations like cytoplasmic vacuolization, lipid inclusion, increased exocytosis activity in L. amazonensis and L. infantum, plus autophagosome-like vesicles in last one. The noni in vitro treatment led to an increased nitrite production in infected and non-infected peritoneal macrophages. When macrophages were pre-treated with aminoguanidine, a selective inhibitor of iNOS, the L. amazonensis intracellular amastigotes increased, demonstrating the important role of NO production in M. citrifolia fruit activity. In vivo treatment with 500 mg/kg of noni for 60 days decreased the lesion size and parasitic load on the C57BL/6 footpad infected with L. amazonensis. The site of infection also showed decreased inflammatory infiltrates and lower expression of IL-12, TNF-\03B1, TGF-\03B2 and IL-10, when compared with infected non-treated mice. On the other hand, noni treatment increased the expression of collagen IV, fibronectin and laminin in infected footpad as well collagen type I and type II, fibronectin and laminin in mock-infected footpad. No toxicity was observed using this treatment. These data show the efficacy of noni treatment in the described model and a future potential for treatment against leishmaniasis.
