Abstract in Portuguese
O presente estudo teve por objetivo identificar quais as penosidades presentes no cotidiano do trabalho de uma equipe da Estratégia Saúde da Família (ESF) do município do Rio de Janeiro, a partir das percepções desses profissionais de saúde sobre o trabalho na atenção primária sob a égide de novos modelos de gestão e gerenciamento do trabalho, implantados no município do Rio de Janeiro após 2009. O contexto do estudo remete ao processo de reestruturação produtiva e aos novos modos de gestão e gerenciamento do trabalho que vem sendo implantados tanto em empresas privadas como em serviços ou empresas públicas. Essas transformações foram incorporadas à dinâmica dos serviços públicos, de acordo com a lógica neoliberal e de reformas administrativas do aparelho de Estado, como resposta à crise capitalista do Estado do Bem-Estar-Social e do modelo de produção hegemônico fordista-taylorista, sob o discurso da promoção da melhor eficiência da máquina pública. No Brasil, essas transformações nos serviços públicos foram sendo executadas durante a década de 1990, como nos serviços de saúde, os quais tinham sofrido mudanças substanciais, principalmente em seu modelo assistencial, com a criação do Sistema Único de Saúde (SUS) e da ESF. Por conseguinte, várias repercussões e contradições dos novos formatos gerenciais vêm se apresentando no cotidiano do trabalho, principalmente no que tange à subjetividade e às experiências vividas pelos trabalhadores no ambiente de trabalho. Com isso se retoma a discussão em torno de aspectos que ressignificam o sentido do trabalho, tornado o mais explorado e mais difícil de ser suportado, gerando o que tem sido denominado pela Sociologia do Trabalho francesa de penosidades. O estudo foi realizado numa clínica da família do Rio de Janeiro, inspirando-se na etnografia de caso ampliado, desenvolvida por Michael Burawoy e utilizou a observação participante com a realização de entrevistas semiestruturadas com trabalhadores de uma equipe da ESF dessa clínica. Após análise do diário de campo e das entrevistas, verificou-se a predominância de mulheres na equipe estudada, correspondendo à tendência da feminização da força de trabalho na saúde. Além disso, foram caracterizadas como penosidades: a jornada de trabalho extensa, a constante interação com os usuários, o excesso de demanda de trabalho, a exigência de polivalência e mudança constante de funções no ambiente de trabalho. Ressalta-se a necessidade de aprofundar o estudo das contradições entre os valores e aspectos que envolvem esse o modelo de gestão e gerenciamento do trabalho com a lógica do cuidado e outros valores preconizados historicamente pela reforma sanitária e na construção do SUS.
Abstract
The present study aimed to identify the workers suffering and pain present in the daily work of a team of the Family Health Strategy (ESF) in the city of Rio de Janeiro, based on the perceptions of these health professionals about work in primary care under the new management and work management models implemented in the city of Rio de Janeiro after 2009. The context of the study refers to the process of productive restructuring and the new management and work management modes that have been implemented both in private companies and in services or public companies. These transformations were incorporated into the dynamics of public services, in accordance with the neoliberal logic and administrative reforms of the State apparatus, as a response to the capitalist crisis of the Welfare State and the fordist-taylorist hegemonic production model with the idea of promoting the best efficiency of the public machine. In Brazil, these transformations in public services were implented during the 1990s, as the health services, which had substantial changes, mainly in its care model, with the creation of the Unified Health System (SUS) and the ESF. Consequently, several repercussions and contradictions of the new management formats have been appearing in the work routine, especially regarding the subjectivity and the experiences lived by the workers in the work environment. The discussion about aspects that re-signify the meaning of work as something more difficult to do has been denominated by the French Sociology as workers suffering and pain. The study was in a “family clinic” of Rio de Janeiro, drawing on the case-study ethnography developed by Michael Burawoy and used the participant observation with the semi-structured interviews with workers of an ESF team of this clinic. After analyzing the field diary and the interviews, the predominance of women in the study team was verified, corresponding to the feminization trend of the health workforce. In addition, some situations were characterized as workers suffering and pain: the extensive working day, the constant interaction with the users, the excess of work demand, the requirement of polyvalence and constant change of functions in the work environment. It is necessary to deepen the study of the contradictions between the values and aspects that involve this model of management and work management with the logic of care and other values historically advocated by health reform and the construction of SUS.
