Durante a 14ª Conferência Nacional de Saúde no Brasil, a professora Maria Inês de Sousa Bravo destacou a existência de dois projetos de saúde pública em disputa: a Reforma Sanitária dos anos 80 e um projeto privatista ligado ao mercado. Ela também mencionou um terceiro projeto, chamado de "Reforma Sanitária flexibilizada", criticando ex-companheiros de lutas. O evento foi marcado por críticas recíprocas entre defensores dessas abordagens.
Um grupo vê o aumento da procura como algo positivo, pois encarece a oferta, enquanto outros enfrentam obstáculos devido à influência persistente do neoliberalismo no Brasil e no mundo. Alguns sanitaristas, cansados ou modernizados em suas lutas, passaram a formar um terceiro grupo, apoiando propostas que flexibilizam princípios da Reforma Sanitária. Esse terceiro grupo, embora critique o mercado e o macro modelo de desenvolvimento, parece hesitar em derrubar o conjunto de leis, interpretações jurídicas e práticas administrativas ligadas ao capital financeiro. A Lei de Responsabilidade Fiscal e outras práticas neoliberais foram apontadas como obstáculos à expansão dos serviços públicos, como saúde, educação e segurança. A maioria dos delegados à conferência expressou descontentamento com a terceira via, acusando-a de se voltar contra direitos, conquistas e princípios da Reforma Sanitária. Propostas consideradas flexibilizadoras e privatizantes são rejeitadas nos fóruns democráticos do Sistema Único de Saúde (SUS). O resultado da conferência destaca a necessidade de a terceira via se qualificar melhor para apresentar alternativas aceitáveis ou realinhar-se com a defesa renovada dos princípios da Reforma Sanitária contra as adversidades do mercado e do capital.
