Resumen en portugues
Este estudo analisa o projeto de instalação da Sociedade Moçambicana de Medicamentos S.A., uma
cooperação internacional realizada entre Brasil e Moçambique, executada pelo Instituto de
Tecnologia em Fármacos (Farmanguinhos), da Fundação Oswaldo Cruz (Fiocruz). O objetivo dessa
cooperação é transferir tecnologia para a produção pública de medicamentos no país, com o propósito
de apoiar a ampliação do acesso da população aos medicamentos, sobretudo antirretrovirais.
Consideraram-se especialmente os aspectos operacionais do projeto. Trata-se de um estudo de caso,
alicerçado na análise bibliográfica e documental, entrevistas e observação direta. A hipótese
norteadora foi que projetos de transferência de tecnologia para a produção de medicamentos em
países em desenvolvimento podem contribuir para a melhoria do acesso da população local a tais
insumos, desde que outros pré-requisitos sejam observados, tanto no país que oferta a cooperação
quanto no país receptor, tais como, mecanismos apropriados que viabilizem a cooperação, mão de
obra qualificada, conjunturas políticas e econômicas favoráveis. Os resultados evidenciam que o
Brasil conseguiu transferir a tecnologia e instalar a estrutura física necessária à produção de
medicamentos em Moçambique; capacitar profissionais; efetivar a produção de parte da lista de
medicamentos acordada com o Ministério da Saúde de Moçambique; viabilizar a obtenção dos
registros para a produção de medicamentos junto à autoridade regulatória local; impulsionar a revisão
da legislação moçambicana sobre medicamentos, levando em consideração as necessidades da nova
indústria farmacêutica; e apoiar o efetivo funcionamento da fábrica. Esse processo trouxe ganhos para
Brasil: aprendizado da equipe envolvida no projeto e acúmulo de conhecimentos, em
Farmanguinhos/Fiocruz, sobre o planejamento, avaliação e execução de atividades de cooperação
internacional focadas na transferência de tecnologia para a produção farmacêutica. Estimulou a
reflexão crítica sobre as potencialidades e fragilidades da prática brasileira de cooperação
internacional nessa área, bem como discussões sobre os mecanismos necessários para a
implementação desse tipo de empreendimento em países em desenvolvimento. Por outro lado, várias
dificuldades tiveram que ser superadas, em ambos países, e inúmeros problemas ainda permanecem
para a efetiva operação autônoma da fábrica. Entre eles destacam-se: pouca quantidade e experiência
de profissionais locais para atuar numa indústria farmacêutica; dificuldade de retenção da mão de
obra capacitada pelo projeto; pequeno número, no país, de fornecedores de insumos e de prestadores
de serviços especializados no segmento farmacêutico; alta dependência nacional da ajuda externa e
de doações de medicamentos; falta de consenso no governo sobre a importância da produção pública
nacional de medicamentos; e ausência de uma política nacional de assistência farmacêutica que
valorize a produção local de medicamentos como um dos itens fundamentais para suprir as
necessidades da população e impulsionar o desenvolvimento do país.
Resumen en ingles
This study examines the project to set up the Sociedade Moçambicana de Medicamentos (SMM),
involving international cooperation in health pursued by Brazil and Mozambique and carried out by
the Instituto de Tecnologia em Fármacos (Farmanguinhos) of the Fundação Oswaldo Cruz (Fiocruz).
The goal of this cooperation is to transfer technology for public drug production in Mozambique, in
support of the country’s endeavours to expand its population’s access to drugs, particularly
antiretrovirals. The research is a case study drawing on bibliographical and documentary analysis,
interviews and direct observation, focussed especially on operational aspects of the project. The
underlying hypothesis was that, in developing countries, drug production technology transfer projects
can contribute to improving the local population’s access to such products, providing that other
prerequisites of effective cooperation are met, either in the country offering the cooperation or in the
recipient country. These include appropriate mechanisms to operationalise the cooperation, skilled
labour and favourable political and economic conjunctures. The results show that Brazil has managed:
to transfer the technology and install the physical structure necessary to produce drugs in
Mozambique; to train personnel; to operationalise production of part of the list of drugs agreed with
the Ministry of Health (MISAU); to enable registration to be obtained for drug production from the
Mozambican regulatory authority; to foster review of local legislation on drugs, in view of the needs
of the new pharmaceutical industry; and to provide support for the plant to function effectively. This
process has yielded gains for Brazil: learning by the project team and an accumulation of knowledge
at Farmanguinhos/Fiocruz on the planning, evaluation and execution of international cooperation
activities for drug production technology transfer. It has stimulated critical thinking on the potentials
and fragilities of pursuing international cooperation in this field and stimulated discussions of the
mechanisms necessary in order to implement this type of undertaking in developing countries. On the
one hand, various difficulties had to be surmounted, in both countries, and numerous problems still
remain in order for the plant to operate effectively with autonomy. Prominent among these are: there
is a shortage of experienced local personnel to work in a pharmaceutical industry; labour trained by
the project is difficult to retain; few materials suppliers and service providers in Mozambique
specialise in the pharmaceuticals industry; Mozambique is highly dependent on external aid and drug
donations; there is no government consensus on the need for public production of medicines in
Mozambique; and there is no national pharmaceutical assistance policy supporting local drug
production as an item fundamental to the country’s development and to meeting its population’s
needs.
