Esta tese busca contribuir para a melhoria da segurança dos cuidados de saúde em hospitais brasileiros de pacientes agudos, a partir do desenvolvimento de indicadores de segurança do paciente. Os resultados estão apresentados em três artigos. O primeiro inclui uma revisão sistemática da literatura realizada com a finalidade de conhecer as estratégias utilizadas no desenvolvimento de indicadores de segurança do paciente, para analisar os modelos conceituais que orientam esses processos, os métodos e critérios utilizados para selecionar e validar os indicadores e definir uma lista de indicadores. O segundo artigo apresenta o processo de planejamento e aplicação do Delphi, método escolhido para que profissionais, pesquisadores/professores e gestores de saúde, das várias regiões do Brasil, avaliassem e selecionassem, com base em consenso, indicadores de segurança do paciente relacionados ao controle de infecção e uso de medicamentos. São descritos os passos percorridos desde a identificação dos indicadores, a escolha do método de consenso mais apropriado aos objetivos e sua execução utilizando a Internet como veículo de comunicação. O terceiro artigo apresenta a execução do método Delphi e os resultados obtidos, com destaque para a lista de indicadores selecionados pelos grupos de especialistas. Na revisão sistemática foram identificados 285 indicadores, sendo 160 (56 por cento) classificados apenas na dimensão da qualidade segurança. A maioria dos indicadores identificados foi sobre uso de medicamentos (22 por cento) e controle de infecção (15 por cento). A aplicação do Delphi através da Internet apresentou uma taxa de resposta de 27 por cento na primeira rodada do método e de 90 por cento na segunda. Oitenta e sete especialistas concluíram as duas rodadas do método (32 nos grupos de controle de infecção e 55 nos de uso de medicamentos) e selecionaram 47 indicadores de segurança do paciente, 23 sobre controle de infecção e 24 de uso de medicamentos.
A aplicação do Delphi através da Internet se mostrou efetiva para reunir um número importante de especialistas das várias regiões do país e de diferentes formações profissionais, auxiliando a composição de grupos multidisciplinares e conferindo credibilidade aos resultados. Há necessidade de se estudar estratégias para aumentar a adesão dos profissioais a pesquisas que utilizem este tipo de abordagem. Os indicadores selecionados englobam eventos e processos importantes relacionados à segurança do paciente, como as infecções de sítio cirúrgico e as falhas nas prescrições de medicamentos, e representam um passo importante na identificação de indicadores válidos e viáveis que possam ser utilizados em nível nacional. Há um caminho a ser percorrido para o refinamento dos indicadores, o que poderia ser realizado por painel de especialistas, utilizando metodologia presencial, como o grupo nominal, ou usando recursos audiovisuais, como a teleconferência, em uma etapa posterior à aplicação do método Delphi, e anterior a um estudo piloto.
