A leishmaniose tegumentar americana é uma doença que acomete a pele e as mucosas das vias aerodigestivas superiores. Os antimoniais pentavalentes vêm sendo empregados há muitas décadas como fármacos de primeira linha para o seu tratamento. Pacientes com poucas lesões cutâneas, com impossibilidade de receber medicação parenteral regular ou com sinais de toxicidade importante ao antimonial por via sistêmica, podem ser submetidos ao tratamento intralesional com antimoniato de meglumina. Objetivos: descrever a eficácia e a segurança do antimoniato de meglumina administrado por via intralesional, para o tratamento da leishmaniose cutânea. Método: Foi realizado um estudo descritivo, retrospectivo, tipo série de casos, de pacientes atendidos no Laboratório de Vigilância em Leishmanioses do Instituto de Pesquisa Clínica Evandro Chagas \2013 FIOCRUZ, de 2002 até julho de 2011, que tivessem sido tratados para leishmaniose cutânea com aplicação intralesional de antimoniato de meglumina, após tratamento sistêmico com o mesmo fármaco
Além disso, relatamos dois casos especiais de pacientes tratados com antimoniato de meglumina intralesional: o primeiro, de um paciente que apresentou resistência ao antimoniato de meglumina sistêmico e intralesional, não tolerou anfotericina B e respondeu apenas à pentamidina. O outro, de paciente com eczema generalizado após antimoniato de meglumina sistêmico e que desenvolveu eczema bolhoso em local de aplicação intralesional do mesmo fármaco. Resultados: Pacientes com leishmaniose cutânea apresentaram boa resposta terapêutica ao antimoniato de meglumina administrado por via intralesional com um mínimo de efeitos adversos, geralmente sem necessidade de mudar o fármaco para outros de mais difícil administração e alto custo, e sem desenvolvimento de lesões mucosas. Medicamentos de segunda ou terceira escolha são eficazes, na ausência de resposta prévia ao antimoniato de meglumina sistêmico e intralesional. A via intralesional deve ser evitada em caso de farmacodermia ao antimoniato de meglumina sistêmico.

American tegumentary leishmaniasis is a disease that affects the skin and mucous membranes of the upper aerodigestive tract. Pentavalent antimonial compounds have been used for decades as a first - line drugs for its treatment. Patients with few skin lesions, with inability to receive parenteral medication regularly or with signs of significant toxicity to antimony systemically, may be subjected to treatment with intralesional meglumine antimoniate. Objectives: To describe the efficacy and safety of meglumine antimoniate administered intralesiona lly, for the treatment of cutaneous leishmaniasis. Methods: We conducted a descriptive, retrospective case series of patients treated at the Leishmaniasis Surveillance Laboratory, Evandro Chagas Clinical Research Institute, Oswaldo Cruz Foundation, Rio de Janeiro, Brazil , from 2002 until July 2011, which had been treated for cutaneous leishmaniasis with intralesional meglumine antimoniate after systemic treatment with the same drug. In addition, we report two special cases of patients treated with intralesi onal meglumine antimoniate: the first, a patient who was resistant to systemic and intralesional meglumine antimoniate, did not tolerate amphotericin B and answered only to pentamidine. The other one reports a patient with generalized eczema after systemic meglumine antimoniate and moderate eczema at the place of intralesional injection of the same drug. Results: Patients with cutaneous leishmaniasis showed good therapeutic response to intralesional meglumine antimoniate, with a minimum of adverse effects, generally without changing the drug to other one of more difficult and costly administration, without development of mucosal lesions. Drugs of second or third choice are effective in the absence of prior response to systemic meglumine antimoniate and intra lesional. The intralesional route should be avoided in cases of pharmacodermia after systemic meglumine antimoniate.
