Existem indícios de que o benzonidazol interfere diretamente nos padrões imunológicos do hospedeiro tratados na fase crônica da doença de Chagas. Contudo, ainda não foi realizado estudo longitudinal com portadores crônicos assintomáticos e sintomáticos submetidos ao tratamento etiológico onde fossem avaliados parâmetros imunológicos relacionados à ativação, citotoxicidade, regulação e inflamação celular de forma simultânea. Diante disto, nos propomos a investigar a resposta imune de portadores crônicos da doença de Chagas antes a após o tratamento com o benzonidazol. Foram selecionados vinte e um adultos classificados na fase crônica da doença de Chagas (forma indeterminada-IND=13/forma cardíaca-CARD=8). Amostras de sangue foram coletadas antes do tratamento etiológico (T0/n=21), após 2-3 meses (T1/n=21) e 5-6 meses (T2/n=16), sendo avaliadas por citometria de fluxo. Observamos um aumento de linfócito B1 CD5+CD19+, bem como uma maior ativação de linfócitos T CD8+ citotóxicos (CD28+), indicando que o benzonidazol exacerbou mecanismos celulares relacionados à eliminação do Trypanosoma cruzi. Contudo, houve redução em todos os indivíduos de linfócitos T e B produtores de IFN-gama+. Tais fenômenos, associados ao baixo percentual de linfócitos T CD4+CD25+/highFoxP3+, demonstram perda de mecanismos de combate ao parasita e de controle da resposta inflamatória. Porém, ressaltamos que o surgimento de um padrão de citocinas Th1 modulado (inflamatória: IFN-gama/ regulatórias: IL-4, IL-6, IL-10) de forma tardia (T2) no grupo IND+CARD pode indicar que, após um inicial desequilíbrio imune, o organismo do hospedeiro estaria migrando para uma imunomodulação. Desta forma, o benzonidazol causou um desequilíbrio de diferentes mecanismos relacionados à resposta imune na doença de Chagas crônica. Porém, apenas o acompanhamento por um maior intervalo de tempo possibilitará concluirmos se estes fenômenos são definitivos ou se são consequência dos elevados níveis antigênicos e medicamentosos no organismo do hospedeiro.
