A doença de Chagas (DC) é uma infecção causada pelo Trypanosoma cruzi, é considerada endêmica na América Latina, afetando cerca de 15 milhões de indivíduos. Estima-se que cerca de 30 por cento das pessoas infectadas desenvolvem cardiomiopatia chagásica crônica, entre 5 à 30 anos após a infecção aguda. Com o objetivo de diferenciar portadores de DC com a evolução potencial para formas clínicas crônicas graves, pesquisadores tentam estabelecer marcadores biológicos de prognóstico da evolução da doença por meio de marcadores imunológicos. Lectina de Ligação a Manose (MBL) é uma molécula de reconhecimento de que a imunidade inata que desempenha um papel fundamental na defesa do hospedeiro, mediando a fagocitose e a destruição dos agentes patogénicos mediada pelo complemento. Existem vários estudos que enfatizam a relevância da MBL em diferentes doenças infecciosas, inflamatórias e auto-imunes. A deficiência de MBL pode implicar na susceptibilidade bacteriana, fúngica, por protozoários e infecções virais. Nosso objetivo foi investigar a associação dos níveis séricos e atividade de ligação da MBL com cardiomiopatia chagásica crônica, através da formação de um índice, que inferiu as moléculas ligantes. Para isso, foi feita uma avaliação, através de ELISA, dos níveis séricos e da capacidade de ligação da MBL, para formação desse índice de relação (Mbi), em pacientes crônicos assintomáticos e cardíacos da doença de Chagas. O estudo incluiu 77 pacientes portadores DC indeterminados (n = 19), cardíaco grave (n = 29) e cardíaco leve (n = 29). Não foi observada diferença significativa nos níveis séricos de MBL entre os grupos de pacientes estudados. No entanto, em relação a atividade de ligação da MBL, houve diferença estatística entre indeterminados e cardíacos leves (p=0,02) e indeterminados e cardíacos graves (p=0,03). Com a formação do Mbi a comparação entre todos os grupos tiveram diferença estatística significante: indeterminados versus cardíacos leves p=0,02; cardíacos leves versus cardíacos graves p=0,01 e indeterminados versus cardíacos grave p < 0,0001. Com base nesses resultados concluímos que, a MBL é um possível marcador para a gravidade da cardiomiopatia chagásica crônica, visto que, o índice (Mbi) criado nesse estudo foi correlacionado com as diferentes formas estudadas. Novos estudos são necessários para confirmar da MBL como indicador da evolução/progressão para as formas mais graves da doença, para assim contribuir com a melhora na qualidade de vida dos pacientes com doença de Chagas crônica.
Abstract
The Chagas disease (CD) is an infection caused by Trypanosoma cruzi. This disease is
considered endemic in Latin America, affecting approximately 15 million individuals. It is
estimated 30% of those infected develop chronic Chagas cardiomyopathy, between 5 to 30
years after the acute infection. Aiming to differentiate CD’s bearers with potential evolution
to chronic severe clinical forms, researchers try to establish biological markers in order to
prognostic disease evolution through immunological molecules. The Mannose Binding
Lectin (MBL) is a innate immunity recognition molecule that plays a crucial role in host
defense, mediating the phagocytosis and complement-mediated destruction of pathogens.
Several studies have emphasized the importance of MBL in various infectious, inflammatory
and autoimmune diseases and some of these have shown that MBL deficiency might be
involved in susceptibility to bacterial, fungal, protozoal and viral infections. The aim of this
study was to investigate the association of MBL serum levels and binding activity with
chronic Chagas cardiomyopathy, by forming an index that inferred the linker molecules.
Therefore, an assessment by ELISA of the MBL serum levels and binding capacity was made
for formation of the Mannose binding index (Mbi) in chronic patients with asymptomatic and
cardiac forms of CD. This study included 77 patients with CD indeterminate (n=19), severe
cardiac (n=29) and mild cardiac forms (n=29). There was no significant difference in serum
MBL levels between groups of patients. However, evaluating the Mannose binding activity,
there was a statistical difference between indeterminate and mild cardiac groups (p=0.02) and
severe cardiac and indeterminate groups (p=0.03). With the formation of Mbi, the comparison
between all groups had statistically significant difference; indeterminate versus mild cardiac
(p=0.02); mild cardiac versus severe cardiac (p=0.01) and indeterminate versus severe cardiac
(p<0.0001). Based on these results, we found a correlation with the different forms studied.
So we suggest that MBL is able to be an immunological marker for the severity of chronic
Chagas cardiomyopathy. New studies are necessary to confirm the MBL use as an indicator of
the development/progression to more severe forms of the disease, contributing to improve the
life quality of patients with chronic Chagas disease.
