Os indivíduos afetados por trombofilia adquirida ou hereditária têm mais propensão a tromboembolismo venoso e arterial. Portadores de anemia falciforme (AF) são suscetíveis a episódios trombóticos e têm nas crises vaso-oclusivas (CVO) a principal manifestação clínica, com gravidade variável entre pacientes. Identificar marcadores de trombofilia em portadores de AF com relação aos eventos vaso-oclusivas (EVO): síndrome torácida aguda (STA), acidente vascular cerebral isquêmico (AVC), necrose asséptica de cabeça de fêmur (NACF), priapismo, síndrome mão-pé (SMP) e CVO. Após consentimento informado, foi coletada amostra de sangue de 100 indivíduos portadores de AF, em steady state, e estudadas as variáveis: faixa etária, sexo, os marcadores de trombofilia: anticorpos antifosfolipídios - anticardiolipina (ACA), anti?2 glicoproteína I (A?2GPI), antifosfatidilserina (aPS), antilúpico (AL) -, Fator V de Leiden (FVL), Fator II G2021A (PM), polimorfismo C677T da MTHFR (MTHFR), deficiência de antitrombina (AT) e FVIII elevado. Idade e sexo não apresentaram associação com EVO; idade média do grupo estudado foi de 19,8 anos; 55 por cento eram do sexo feminino e 45 por cento eram do sexo masculino. Trombofilia foi identificada em 62 por cento e mostrou associação significante com CVO, enquanto A 2GPI mostrou associação com AVC. MTHFR apresentou associação significante para CVO. A deficiência de AT aumentou o risco para SMP, sem significância estatística. FVIII elevado apresentou associação significante com EVO. Apenas 1 (1,15 por cento) foi heterozigoto para o FII G20210A, enquanto que FVL não foi identificado. A 2GPI estão associados com risco para AVC na população em geral. A virtual ausência do FVL e do FII G20210A indica que não representam risco para complicações na AF na população estudada. A deficiência de AT sugere ativação da coagulação na AF, apesar de terem sido selecionados pacientes fora de crise. FVL e PM não são indicados como marcadores de risco para EVO, ou mesmo de TEV na AF. Recomenda-se pesquisa do A 2GPI IgG para risco de AVC, do polimorfismo C677T da MTHFR para risco de CVO e dosagem do fator VIII para EVO na AF
