Introdução: A determinação do estágio de fibrose hepática tem implicações terapêuticas, no monitoramento e prognóstico de pacientes com hepatites virais e/ou infecção pelo HIV. A biópsia hepática é exame padrão-ouro para estadiamento da fibrose hepática, porém é um método invasivo, com limitações/complicações. A elastografia hepática transitória (EHT) por Fibroscan é o método não-invasivo mais validado para estadiamento da fibrose hepática. A elastografia hepática por 2D-SWE (EH-2D-SWE) permite a avaliação da morfologia hepática/dopplerfluxometria do sistema portal e o estadiamento da fibrose hepática. Poucos estudos avaliaram acurácia da EH-2D-SWE e sua correlação comparada à EHT. Objetivos: Avaliar a acurácia do desempenho diagnóstico da EH-2D-SWE em pacientes com hepatites virais e/ou infecção pelo HIV e descrever os pontos de corte da EH-2D-SWE para estadiamento da fibrose e cirrose hepática. Métodos: Estudo retrospectivo transversal, de um banco de dados de 348 pacientes com hepatites virais e/ou infecção pelo HIV, que realizaram EHT(Fibroscan) pelas sondas M e XL e EH-2DSWE (LOGIQ-S8), pelo mesmo examinador. A EHT foi o padrão-ouro e o estadiamento da fibrose hepática foi definido pela “regra dos 5” [≥5,0KPa, ≥10,0kPa e ≥ 15,0KPa], proposta no consenso de Baveno VI. Foram coletados resultados de exames laboratoriais até 6 meses da data da EH. Análises de amostras pareadas (Wilcoxon), correlação (Spearman) e a descrição da área sob a curva ROC, foram realizadas para avaliar a correlação e a acurácia Resultados: 305 pacientes [61,3% do sexo masculino, mediana de idade de 51 (IQR, 42-62) anos, 24% com HCV±HIV; 17% com HBV±HIV; 31% com HIV monoinfectado e 28% com HCV±HIV após resposta virológica sustentada foram incluídos. O coeficiente de correlação [Spearman’s rho] entre EHT-M e EH-2D-SWE foi moderado [rho=0.639] e fraco entre EHT-XL e EH-2D-SWE [rho=0.566]. A concordância foi forte nos pacientes com HCV ou HBV mono-infectados [rho>0.800] e fraca nos pacientes HIV monoinfectados [rho>0.400]. A acurácia da EH-2D-SWE para EHT-M≥ 10 kPa [AUROC=0.91 (95%CI,0.86-0.96); o melhor ponto de corte foi 6.4 kPa, sensibilidade (Se)=84% (95%CI,72-92), especificidade (Sp)=89% (95%CI, 84-92)] e para EHT-M≥ 15 kPa [AUROC=0.93 (95%CI,0.88-0.98); o melhor ponto de corte foi 7.1 kPa, (Se)=91% (95%CI, 75-98), (Sp)=89% (95%CI,85-93)]. Conclusão: A EH-2D-SWE LOGIQ S8 apresentou valores de rigidez hepática inferiores à EHT e a correlação entre os métodos foi boa. Para pacientes monoinfectados por HIV a correlação foi fraca. A EH2D-SWE demonstrou ter boa acurácia para detecção de estágios avançados de fibrose
Abstract
Background: The evaluation of the degree of liver fibrosis has important implications for prognostic, therapeutic and monitoring purposes in patients with viral hepatitis and/or HIV infection. Liver biopsy is the gold standard for staging liver fibrosis, despite its invasiveness, potential complications and sampling errors. Liver elastography methods are an alternative to liver biopsy. Transient Elastography (TE) by FibroScan is the most validated non-invasive method for staging liver fibrosis. New technologies of real time hepatic elastography, such as 2D Shear Wave Elastography (2D-SWE), allow the staging of liver fibrosis and the evaluation of liver morphology/dopplerflowmetry, with a B-mode ultrasound. However, this method still needs further validation by clinical studies. Aims: We aimed to validate the diagnostic performance of 2D-SWE and propose new cutoff points for the staging of liver fibrosis in patients with chronic viral hepatitis and/or HIV infection. Methods: This retrospective study analyzed a database of 348 patients with chronic viral hepatitis and/or HIV infection who underwent TE (FibroScan), using M probe and XL probe, and 2DSWE(LOGIQ-S8), on the same day by a single operator. TE was considered the gold standard and the results were converted to a simplified classification called “the rule of 5” [≥ 5,0 KPa, ≥ 10,0 KPa e ≥ 15,0 KPa], proposed in Baveno VI Consensus. Blood samples were collected up to six months before the elastography exams. The correlation of liver stiffness was assessed using Spearman rank correlation. The agreement was assessed by Wilcoxon test. The accuracy of 2D-SWE was evaluated with areas under the ROC curves (AUROC) Results: 305 patients [61.3% male, median age=51 (IQR,42-62) years, 24% with HCV±HIV; 17% with HBV±HIV; 31% were HIV mono-infected and 28% had HCV±HIV post-sustained virological response] were included. The overall correlation [Spearman’s rho] was moderate between 2DSWE and TE-M [rho=0.639] and weak between 2D-SWE and TE-XL [rho=0.566]. Agreements were strong [rho>0.800] in people with HCV or HBV mono-infection, and poor in HIV mono-infected [rho>0.400]. Accuracy of 2D-SWE for TE-M ≥ 10 kPa [AUROC=0.91 (95%CI,0.86-0.96); optimal cut-off=6.4 kPa, sensitivity (Se)=84% (95%CI,72-92), specificity (Sp)=89% (95%CI, 84-92)] and for TE-M ≥ 15 kPa [AUROC=0.93 (95%CI,0.88-0.98); optimal cut-off=7.1 kPa. Se=91% (95%CI, 75-98), Sp=89% (95%CI,85-93)] were excellent. Conclusions: 2D-SWE LOGIQ S8 system showed lower LSM results when compared with TE. The correlation and agreement between 2D-SWE and TE-M were good. In HIV monoinfected patients, the correlation was weak. Finally, 2D-SWE proved to have an excellent accuracy for detecting advanced stages of liver fibrosis
