A Hepatite C é um importante problema de saúde pública no Brasil. De 2000 a 2020 foram notificados 279.872 casos e identificados 62.611 óbitos associados à doença no País. A assistência à saúde da Hepatite C, no Brasil, é realizada gratuitamente pelo Sistema Único de Saúde, organizada de maneira regionalizada pela Rede de Atenção à Saúde, dividida em 117 Macrorregiões de Saúde. Os dados sobre os casos notificados foram obtidos através do Departamento de Informática do Sistema Único de Saúde, de 2015 a 2020. Para a análise espacial dos casos, foram calculadas as taxas de detecção das 117 macrorregiões de saúde por ano, suavizadas pelo método da Estimativa Bayesiana Empírica Local. A avaliação da autocorrelação espacial utilizou o Índice local de Moran, por meio do Index of Spatial Association – LISA. São aqui apresentados os resultados estatisticamente significantes (p<0,05). Foram notificados 117.396 casos de Hepatite C em pessoas acima de 18 anos no Brasil, no período. O resultado no LISA se mostrou estatisticamente significativo no quadrante “low-low”, nas macrorregiões da região Nordeste e nas macrorregiões do Norte e Noroeste de Minas Gerais, em todos os anos avaliados. Em contrapartida, na Região Sul, observou-se resultado positivo no quadrante “high-high” nas diferentes macrorregiões em todos os anos analisados. Apenas uma macrorregião no estado de Minas Gerais, apresentou achados situados no quadrante “low-high”, nos anos 2015-2017. As macrorregiões do Norte e Centro-Oeste do País não geraram resultados estatisticamente significantes, ainda que considerando um p-valor generoso frente à magnitude do universo pesquisado. Para a alcançar a eliminação da Hepatite C como problema de saúde pública é necessário contar com um diagnóstico oportuno, portanto, se torna urgente o fortalecimento da vigilância em saúde nas macrorregiões de saúde nas regiões Norte, Nordeste, Centro-Oeste cujas taxas de detecção se mostraram nulas ou negativas, a fim de assegurar o acesso e a integralidade da assistência à saúde da população.
Abstract
Hepatitis C is an important public health problem in Brazil. From 2000 to 2020, 279,872 cases were reported and 62,611 deaths associated with the disease have been identified in the country. Health care for Hepatitis C in Brazil is provided free of charge by the Unified Health System, organized regionally by the Health Care Network, divided into 117 Health Macroregions. Data on reported cases were extracted from the database of the Department of Informatics of the Unified Health System, 2015-2020. For the sake of spatial analysis of cases, detection rates were calculated for the 117 health macro-regions per year, smoothed by the Local Empirical Bayesian method. Moran's Local Index has been used to assess spatial autocorrelation, taking the Index of Spatial Association – LISA as the key statistics, Statistically significant results at the level of p<0.05 were presented. A total of 117,396 cases of Hepatitis C were reported in people over 18 years of age in Brazil during the period under analysis. The LISA index was statistically significant in the quadrant “low-low” in the macro-regions of the Northeast region and in the macro-regions of the North and Northwest of Minas Gerais, in all the years under analysis. On the other hand, in the South Region, there was a positive result in the quadrant “high-high” in all macro-regions in the same period.The single exception was a unique macro-region in the state of Minas Gerais, where findings were located in the low-high quadrant, in the years 2015-2017. The macro-regions in the North and Midwest of the country did not reach statistically significance, even considering a relaxed assumption (p<0.05) vis-àvis the magnitude of the universe under analysis. In order to achieve the elimination of Hepatitis C as a public health problem, it’s sorely needed to get timely diagnosis. Therefore, it is urgent to strengthen health surveillance in the health macro-regions in the North, Northeast and Midwest regions, were detection rate assessments yelded zero or negative results. Such step is key to ensure full access and comprehensiveness of health care offered to this population.
