O IMPACTO DA TELESSAÚDE DURANTE A PANDEMIA DA COVID-19 NAS CONDIÇÕES CLÍNICAS E NUTRICIONAIS DE ADOLESCENTES COM FIBROSE CÍSTICA

A fibrose cística (FC) é uma doença genética grave, progressiva e multissistêmica que requer uma rotina regular de tratamento presencial. Em 2020, com a pandemia da COVID-19, dada a situação de quarentena imposta pelas autoridades, o autocuidado foi incentivado e os serviços foram prestados remotamente, estabelecendo-se assim uma rotina de teleconsultas multiprofissionais. Diante disso, buscou-se verificar os impactos da telessaúde durante a pandemia de COVID-19 refletidos nas características clínicas e nutricionais de adolescentes acompanhados em um Centro de Referência em Fibrose Cística (CRFC) no estado do Rio de Janeiro (RJ). Foi realizado um estudo longitudinal retrospectivo nos ambulatórios especializados de um CRFC do RJ, onde foram avaliados dados de adolescentes com 12 anos ou mais com diagnóstico de FC. As variáveis estudadas foram: Índice de Massa Corporal (IMC), Estatura para Idade (E/I), percentil de Circunferência Muscular do Braço (pCMB), percentil do Índice de Massa Corporal para Idade (pIMC/I), percentil do previsto do Volume Expiratório Forçado previsto no 1º segundo (ppVEF1), percentil do previsto da Capacidade Vital Forçada (ppCVF) e relação VEF1/CVF, coletados nos anos de 2017, 2018, 2019, 2021 e 2022. Foram avaliados 35 adolescentes com idade média de 11,0 ± 2,7 anos, sendo 54,3% do sexo feminino. Durante o período estudado, foi observado declínio nos parâmetros pulmonares e nutricionais ao longo dos anos. No entanto, esta diminuição não foi agravada entre o período anterior e posterior à interrupção dos serviços presenciais. Este resultado pode ser atribuído à rápida implementação da teleconsulta multidisciplinar no CRFC.
Abstract
Cystic Fibrosis (CF) is a severe, progressive and multisystemic genetic disease that requires a regular, in-person treatment routine. In 2020, with the COVID-19 pandemic, given the quarantine situation imposed by the authorities, self-care was encouraged and services were provided remotely, thus establishing a routine of multi-professional teleconsultations. Given this, we sought to verify the impacts of telehealth during the COVID-19 pandemic reflected in the clinical and nutritional characteristics of adolescents monitored at a Cystic Fibrosis Reference Center (CFRC) in the state of Rio de Janeiro (RJ). A retrospective longitudinal study was carried out in the specialized outpatient clinics of a CFRC in RJ, where data from adolescents aged 12 years or over diagnosed with CF were evaluated. The variables studied were: Body Mass Index (BMI), Height for Age (H/A), Arm Muscle Circumference percentile (pAMC), percentile of Body Mass Index for Age (pBMI/A), percentile of predicted Forced Expiratory Volume in the 1st second (ppFEV1), percentile of predicted of Forced Vital Capacity (ppFVC) and FEV1/FVC ratio, collected in the years 2017, 2018, 2019, 2021 and 2022. 35 adolescents with a mean age of 11.0 ± 2.7 years were evaluated, 54.3% of whom were female. During the period studied, a decline in lung and nutritional parameters was observed over the years. However, this decline was not exacerbated between the period before and after the interruption of in-person services. This result can be attributed to the rapid implementation of multidisciplinary teleconsultation at CRFC.
