ALTERAÇÕES OFTALMOLÓGICAS RELACIONADAS À COVID-19 EM PACIENTES DO INSTITUTO NACIONAL DE INFECTOLOGIA EVANDRO CHAGAS-FIOCRUZ E DO HOSPITAL CENTRAL DO EXÉRCITO

Introdução: A Covid-19 é uma patologia infecciosa emergente, com alta transmissibilidade, recentemente descrita, com casos inicialmente detectados na China e que se disseminou a todos os continentes habitáveis. Causada pelo vírus SARS-CoV-2, um novo betacoronavírus, foi declarada como pandemia pela Organização Mundial de Saúde (OMS) em 11 de março de 2020. O quadro clínico caracteriza-se por febre, tosse seca ou produtiva, dor torácica e dispneia com alterações nos exames de imagem e sangue. As alterações oftalmológicas são descritas secundariamente à patologia em diversos pacientes, podendo ser o primeiro sinal da doença. Há relatos de alterações oftalmológicas, porém não se sabe a fisiopatogenia, o padrão oftalmológico e não há evidências suficientes sobre a frequência, o período de surgimento e o tempo de ocorrência das alterações. Objetivo: Identificar as alterações oftalmológicas em pacientes suspeitos de Covid-19 do Instituto Nacional de Infectologia Evandro Chagas (INI) na Fundação Oswaldo Cruz (Fiocruz) e do Hospital Central do Exército (HCE), com diagnóstico laboratorial para SARS-CoV-2 atendidos durante o período dezembro de 2020 a março de 2022 e elaborar um protocolo para rastrear gravidade oftalmológica em unidades de emergência. Método: Estudo seccional de pacientes suspeitos de Covid-19 atendidos no Instituto Nacional de Infectologia Evandro Chagas e no Hospital Central do Exército. Foram coletados dados sociodemográficos e clínicos por meio de questionários, além de consultas oftalmológicas completas. A consulta foi realizada até 30 dias após o diagnóstico ou após a alta hospitalar. Pacientes internados foram avaliados por meio de mapeamento de retina realizado de rotina pela equipe de Oftalmologia do INI-Fiocruz Resultados: Foram examinados 89 pacientes ambulatoriais do INI-Fiocruz e 16 pacientes ambulatoriais do Hospital Central do Exército. Foram avaliados 99 pacientes internados no Hospital de Infectologia Evandro Chagas. O perfil sociodemográfico revelou o predomínio do sexo masculino, raça branca, solteiro, na faixa etária entre 43 a 52 anos, com 3° grau completo e moradores do Rio de Janeiro. Houve predomínio de febre, tosse seca, anosmia e cefaleia. Entre as comorbidades, predominaram hipertensão arterial sistêmica e rinite alérgica. Os sintomas oftalmológicos mais frequentes foram ardência ocular e dor retro orbitária, dentre eles 1 % de estrabismo, 4,8% de diplopia, 18,1% de baixa acuidade visual, 26,7% de fotofobia, 32,4% de hiperemia ocular, 33,3% de dor retro orbitária e 33,3% de ardência ocular. Na análise do exame oftalmológico, ceratite puntata inferior se destacou entre as alterações oftalmológicos com pressão intraocular dentro da faixa de normalidade, sem restrição muscular e fundoscopia sem alteração. Nos internados, 26,3% apresentavam comorbidades, 7,1% apresentaram mapeamento de retina alterado. Conclusão: É necessário o conhecimento técnico das alterações oftalmológicas dos pacientes diagnosticados com Covid-19, visando ao tratamento imediato para evitar sequelas visuais. Apesar da baixa frequência de alterações oftalmológicas nos pacientes diagnosticados com Covid-19, estas podem ser o primeiro sinal de doenças infectocontagiosas. Um protocolo foi criado para uso no setor de emergência para rastreio oftalmológico, incluindo sintomas e sinais característicos, a fim de facilitar o diagnóstico e determinar a gravidade do paciente e a necessidade imediata de encaminhamento ao oftalmologista
Abstract
Introduction: Covid-19 is an emerging infectious pathology, with high transmissibility, re cently described, with cases initially detected in China and which has spread to all habitable continents. Caused by the SARS-CoV-2 virus, a new betacoronavirus, it was declared a pan demic by the World Health Organization (WHO) on March 11, 2020. The clinical picture is characterized by fever, dry or productive cough, chest pain and dyspnea with changes in imag ing and blood tests. Ophthalmological alterations are described as secondary to the pathology in several patients and may be the first sign of the disease. There are reports of ophthalmological alterations, but the pathophysiology, the ophthalmological pattern is not known and there is not enough evidence about the frequency, period of onset and time of occurrence of the alterations. Objective: To identify the ophthalmological alterations in patients suspected of having Covid 19 at the Instituto Nacional de Infectologia Evandro Chagas INI- (FIOCRUZ) and at the Hos pital Central do Exército (HCE), with laboratory diagnosis for SARS-CoV-2 treated during the period of one year and devise a protocol for tracking eye severity in emergency units. Method: Cohort study of suspected patients in Covid-19 at Army Central Hospital (HCE) and Evandro Chagas National Institute of Infectious Diseases (FIOCRUZ). Demographic and clin ical data will be collected through questionnaires and complete ophthalmic consultations. Fol low-up will be carried out in three months after the first consultation, with a complete eye ex amination and complementary eye examinations Results: We examined 89 outpatients from INI-Fiocruz and 16 outpatients from the Central Hospital of the Army. We assessed 99 patients from the Evandro Chagas Infectious Diseases Hospital. The sociodemographic profile revealed a predominance of males, white, single, aged between 43 and 52 years, with high school education, residents of Rio de Janeiro. There was a predominance of fever, dry cough, anosmia, and headache. Among the comorbidities, systemic arterial hypertension and allergic rhinitis predominated. The most frequent ophthalmologic symptoms were ocular burning and retro-orbital pain among them 1% of strabismus, 4.8% of diplopia, 18.1% of low visual acuity, 26.7% of photophobia, 32.4% of ocular hyperemia, 33.3% of retro orbital pain and 33.3 % of eye burning. In the analysis of the ophthalmologic examina tion, inferior punctate keratitis stood out among the ophthalmologic alterations with intraocular pressure within the normal range, without muscle restriction and fundoscopy without alteration. Among those hospitalized, 26.3% had comorbidities, 7.1% had altered retinal mapping. Conclusion: Technical knowledge of the ophthalmological changes of patients diagnosed with COVID-19 is necessary, aiming at immediate treatment to avoid visual sequelae. Despite the low frequency of ophthalmological changes in patients diagnosed with COVID-19, these may be the first sign of infectious diseases. A protocol was created for use in the emergency depart ment for ophthalmic screening, including characteristic symptoms and signs, to facilitate diagnosis and determine the patient's severity and the immediate need for referral to the ophthalmologist
