O Ministério da Saúde (MS) exige uma requisição para emissão de Boletim de Produção Ambulatorial (BPA-I) como solicitação de exames de linfócitos T CD4+ (LT CD4+ ). No Instituto Nacional de Infectologia Evandro Chagas (INI), esses exames são realizados na Plataforma de Laboratório Multiusuário (PLATLAB). O BPA-I sofreu atualização através do Ofício Circular nº 39/2016/CLAB/DDAHV/SVS/MS e foi definido no Protocolo Clínico e Diretrizes Terapêuticas para Manejo da Infecção pelo HIV em Adultos (PCDT) a inclusão de 3 perguntas no campo 31: Dois últimos CD4 >350 céls/mm3?; Paciente assintomático?; Carga viral indetectável? Se a resposta for “sim” para todas, o sistema bloqueia o cadastro. Quando em algum dos itens é marcado “não”, o cadastro é liberado. Essa mudança ocorreu porque dados mundiais sugerem não haver benefícios no monitoramento de LT-CD4+ nas situações já definidas pelo MS. O estudo teve como objetivo a avaliação do quantitativo de exames de LT-CD4+ solicitados pelo INI e pelas unidades externas, antes e após os novos critérios. Foi avaliada a qualidade do preenchimento de 384 BPA-I escolhidos aleatoriamente no ano seguinte às determinações e um questionário foi aplicado para profissionais solicitantes de CD4 com o intuito de saber se eles seguiam e estavam de acordo com os novos critérios A análise se deu com dados estatísticos do Sistema de Controle de Exames Laboratoriais de CD4+ /CD8+ e Carga Viral do HIV (Siscel) em conjunto com as informações de BPA-I. Os resultados mostraram que, embora tenha ocorrido uma redução de 24,6% no quantitativo total de exames após as novas determinações, ao analisar o BPA-I e confrontar com os dados do Siscel, houve uma discordância no preenchimento das informações sobre o CD4 (32,6%) e a carga viral anteriores (22,1%). Quanto ao preenchimento do questionário, 95,2% (20/21) dos participantes afirmaram estar cientes das alterações no BPA-I, e cerca de 14% (3/21) não estavam de acordo. De posse desses dados, buscamos promover a sensibilização dos profissionais através da produção de um folder. Com isso, pretendemos minimizar os problemas relacionados ao preenchimento, para que a solicitação seja feita de forma racional, dentro dos critérios e com os dados completos
Abstract
The Ministry of Health (MS) requires a request to issue an Outpatient Production Bulletin (BPA-I) as a request for tests for CD4+ T-lymphocyte (LT-CD4+ ). At the National Institute of Infectious Diseases Evandro Chagas (INI), these tests are performed on the Multi-User Laboratory Plataform (PLATLAB). The BPA-I was updated through Circular Letter 39/2016/CLAB/DDAHV/SVS/MS and with the inclusion of 3 questions in the field 31 was defined in the Clinical Protocol and Therapeutic Guidelines for the Management of HIV Infection in Adults (PCDT): Two last CD4 >350 cells/mm3? Asymptomatic patient? Undetectable viral load? If the answer is "yes" to all of them, the system blocks the registration. When any of the items is marked “no”, the registration is released. This change occurred because worldwide data suggest that there are no benefits in LT-CD4+ monitoring in situations already defined by MS. The study aimed to assess the number of LT-CD4+ exams requested by INI and external units before and after the new criteria. The quality of filling 384 BPA-I randomly chosen in the year following the determinations was evaluated and a questionnaire was applied to professionals requesting CD4 in order to find out if they followed and were in accordance with the new criteria The analysis took place with statistical data from the CD4+ /CD8+ Laboratory Test Control System and HIV Viral Load (Siscel) together with the BPA-I information. The results showed that, although there was a 24.6% reduction in the total number of tests after new determinations, when analyzing the BPA-I and comparing with the Siscel data, there was a disagreement in filling in the information about CD4 (32.6%) and previous viral load (22.1%).
